Come join the ParkTree Community Health Center Team!</p> <p>ParkTree Community Health Center provides comprehensive and integrative medical, dental, and behavioral health services to the local community in the Pomona and Ontario areas.</p> <p>We are looking for an energetic, professional Nurse Practitioner to join our team.</p> <p>Comprehensive Employee Benefits including:</p> <ul> <li>Comprehensive Medical/Dental/Vision benefits for Full-Time employees </li><li>Paid holidays, vacation, sick and CME time </li><li>Bonus Opportunty </li><li>Continuing Education Benefits, Retirement Benefits, Loan Repayment Opportunities </li></ul> <p>Department: Medical</p> <p>Days and Hours of Work: Full Time, 36 clinical hours, 4 administrative hours</p> <p>Reports to: Chief Medical Officer</p> <p>Pomona Community Health Center dba ParkTree Community Health Center (PCHC) is a non-profit, Federally Qualified Health Center (FQHC) whose mission is to provide preventive and primary care services to the needy in the community.</p> <p>Job Summary: The Nurse Practitioner is an integral part of the clinical care team providing primary care services to the patients and families served by PCHC.</p> <p>Essential Duties:</p> <ul> <li>Provides comprehensive, high quality and cost-effective primary care services to a diverse population in a patient and family centered environment. </li><li>Within the course of a primary care visit prescribe appropriate medications, order necessary screening and diagnostic tests, and confers and refers for specialty care, when necessary. </li><li>Performs health assessments and physical examinations for adults and children, develops patient plans and works with patients/families to develop self-management goals. </li><li>Maintains the safety and security of the patient/family's Personal Health Information and is compliant with HIPAA standards. </li><li>Ensures all proper documentation is present in the patient's medical record. </li><li>Submits billing codes that reflect diagnosis and the nature of the patient visit. </li><li>Functions on a rotating basis as clinician on-call during after-hours. </li><li>Participates in all safety programs which may include assignment to an emergency response team. </li><li>Participates in Quality Improvement Initiatives and provides input on the development of policies and procedures which impact the health delivery service. </li><li>Complies with all mandatory trainings. </li></ul> <p>Job Qualifications and Skills:</p> <ul> <li>Graduate of an approved nursing program as a Nurse Practitioner - Master's preferred. </li><li>Current license in the State of California as a Registered Nurse, Nurse Practitioner </li><li>Specialty certification </li><li>Current DEA license </li><li>Current CPR certification as a healthcare provider </li><li>Proficiency with a EHR </li><li>Bilingual (English/Spanish) - Preferred </li><li>Productivity requirement: 8.5 - 10 patients per four hour session </li><li>Treat all patients and colleagues with dignity and respect. </li><li>Ability to work with diverse populations </li><li>Flexibility with schedule </li></ul> <p>ParkTree Community Health Center (PCHC) is an equal opportunity employer.
