Extensive knowledge of community resources available to seniors & their families. Ability to effectively represent the program to participants, families, outside agencies, community groups, professionals and the general public. Ability to work independently, exercise sound judgment, and establish effective relationships with community members, referral sources and co-workers. Ability to maintain accurate records and to prepare clear and concise reports, correspondence and other written materials. Help complete application on referrals interested in pursuing PACE. Complete enrollment agreement with newly approved applicants. Performs marketing and educational presentations along with performing intake processing and training.</p> </li></ul>
