The Case Management Coordinator facilitates appropriate healthcare outcomes for members by providing health risk assessments, identifying and assisting with accessing benefits and education for members through the use of care management tools and resources.Required QualificationsCandidate must reside in 60632, 60629, 60636, 60609 (Brighton Park, Chicago Lawn, West Englewood, Bridgeport) or surrounding areas2+ years experience in behavioral health, social services or appropriate related field equivalent to program focusConfidence working at home/independent thinker, using tools to collaborate and connect with teams virtuallyAbility to travel up to 50% within a designated geographic area for in-person case management activities as directed by leadership and/or as business needs ariseExcellent analytical and problem-solving skillsEffective communications, organizational, and interpersonal skillsAbility to work independentlyProficiency with standard corporate software applications, including Microsoft Word, Excel, Outlook and PowerPoint, as well as some special proprietary applicationsEfficient and effective computer skills including navigating multiple systems and keyboardingPreferred QualificationsCase management and discharge planning experienceManaged care experienceEducationBachelor's Degree or non-licensed master level clinician required with either degree being in behavioral health or human services (psychology, social work, marriage and family therapy, counseling) (REQUIRED)Anticipated Weekly Hours40Time TypeFull timePay RangeThe typical pay range for this role is:$21.10 - $44.99This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
