This role works one-on-one with individuals in their homes and community settings to help them achieve goals outlined in their Community Habilitation Plan and Individualized Service Plan (ISP).</p> <p>Primary Responsibilities</p> <ul> <li>Provide transportation or accompany individuals to and from community activities. </li><li>Follow goals and activities outlined in the Individualized Service Plan and Community Habilitation Plan. </li><li>Document daily activities, progress, and observations in required service notes. </li><li>Support individuals in developing social skills, recreational involvement, and community relationships. </li><li>Assist individuals and families in identifying skills to build and develop plans to reach those goals. </li><li>Help individuals access social, recreational, and community resources. </li><li>Identify risks for social isolation and develop plans to enhance social connectedness. </li><li>Negotiate schedules and daily activities with individuals and families. </li><li>Submit accurate and timely service documentation and hours worked. </li><li>Follow all action plans and behavioral plans as required. </li><li>Apply knowledge of emergency procedures as outlined by the agency. </li><li>Promote community participation, independence, and positive social interaction. </li><li>Serve as a positive role model. </li><li>Report incidents, concerns, and updates to the Residence Manager. </li><li>Maintain CPR, First Aid, and SCIP-R certifications. </li><li>Perform other duties as assigned. </li></ul>
