You're eager to contribute to the success of Premier Medical Group, embracing the opportunity to travel to different locations within our network as needed.</p> <p>JOB DESCRIPTION</p> <p>JOB SUMMARY</p> <p>This position includes the direct care of patients in coordination with our physicians and clinical support staff, comprehensive health assessment, management of complex disease states, ordering and review of laboratory and diagnostic tests, executing treatment plans, prescribing, authorizing, and monitoring medication regimens. The ideal candidate will have prior experience in Gastroenterology (GI) and be comfortable managing a range of GI conditions in both acute and chronic settings. Familiarity with GI-specific diagnostics, treatments, and procedures is essential. Additionally, the NP will engage in interdisciplinary collaboration by consulting with other departments as appropriate.</p> <p>Strong clinical judgment, excellent communication skills, and a commitment to patient-centered care are essential for success in this role.</p> <p>ESSENTIAL DUTIES AND RESPONSIBILITIES</p> <ul> <li>Works in collaboration with physicians and other health care providers to provide health care across the continuum of care to assigned patients. </li><li>Obtains patient medical, physical, and psycho-social history and performs comprehensive health assessment including physical exam, based on age and history. </li><li>Conducts non-invasive preventive screening procedures based on age and history. </li><li>Identifies medical and health risks and needs. Updates and records changes in health status in EMR. </li><li>Formulates the appropriate differential diagnosis based on history, physical examination and clinical findings. </li><li>Orders and interprets appropriate diagnostic tests and laboratory studies within protocol guidelines. </li><li>Develops and provides a relevant patient education plan. </li><li>Prescribes and administers appropriate pharmacologic and non-pharmacologic therapies. </li><li>Makes appropriate referrals to other health professionals and community agencies. </li><li>Determines the effectiveness of the plan of care through documentation of client care outcomes. </li><li>Reassesses and modifies the treatment plan as necessary to achieve medical and health goals. </li><li>Completes all documentation in EMR in a timely fashion.
