<p>At Jefferson Center, it is our policy and our mission to be inclusive and mindful of the diversity of everyone who comes through our doors. We are passionate about building a community where mental health matters and equitable care is accessible to all races, ethnicities, abilities, socioeconomic statuses, ages, sexual orientations, gender expressions, religions, cultures, and languages.</p> <p>The Adult Outpatient (AOP) program is comprised of several dedicated and passionate clinicians who provide individual and group psychotherapy as well as appropriate case management and/or care coordination to clients ages 18-59. The Enhanced and Transitional Care (ETC) team, a subset of the AOP program, provides additional support via bridge sessions and more frequent check-ins, to those clients over age 18 that are high-risk and/or in need of additional support between regular therapeutic contacts.</p> <p>As part of the AOP team, our ideal candidate will work within a team of skilled clinicians to meet the needs of all adults, treating a wide range of presenting issues and concerns. Clinicians are afforded the opportunity to work autonomously to provide sound clinical care, while also having the opportunity to consult with and make connections within the team and beyond. Additionally, as a member of the ETC team, our ideal candidate will work with ongoing clinicians, crisis center staff, hospital liaisons, and others to provide wrap-around care for higher-acuity clients. Many of these clients have recently discharged from a hospital, and others require additional support between appointments or between intake and their first session. This team also staffs Open Clinic, a "therapy-on-demand" service designed to provide additional support to those requesting it and offers additional support to clients post-hospital discharge.</p> <p>While a conferred master's degree is required for this position, both licensed and unlicensed candidates interested in sharpening and growing their clinical skills are welcome and encouraged to apply. Additionally, this position is considered hybrid, with two days per week spent at the Independence office providing in-person services to AOP clients, and the remainder of the week spent working remotely providing services via Telehealth. While we anticipate this position will be a hybrid model for the long-term, this is subject to change based upon the needs of the team and clients.</p> <p>Education, Knowledge, Skills, and Experience:</p> <ul> <li> <p>Master's degree from an accredited university/college in a related field, required. Degree must be conferred and must be in a subject area applicable for licensure in Colorado.</p> </li><li> <p>License preferred (LPC, LCSW, LMFT, or LP).</p> </li><li> <p>Candidate must be registered with DORA prior to starting the position.</p> </li><li> <p>Experience providing mental health therapy to individuals with severe mental illness; while two years' experience is preferred but not required, the candidate must have some experience providing formal mental health counseling.</p> </li><li> <p>Knowledge of Stages of Change model, Motivational Interviewing techniques, and Cognitive Behavioral Therapy skills preferred. Knowledge of Recovery-based treatment and trauma informed practices required.</p> </li><li> <p>Knowledge of chemical dependency and abuse, as well as experience in crisis intervention, preferred. Proficient assessment and communication skills required.</p> </li><li> <p>Bilingual and skills/experience in integrated health approaches preferred.</p> </li></ul> <p>Essential Duties and Responsibilities:</p> <ul> <li>Engages clients, as well as family members and significant others as appropriate, in strengths-based and trauma informed therapy. Utilizes goal setting to reach agreed-upon outcomes. </li><li>Provides individual and group treatment promoting change and movement through the therapeutic system, following recovery-based practice guidelines. </li><li>Effectively responds to the client/consumer needs and problems, initiates, and maintains positive interactions, and responds to phone calls, email, and other requests in a timely manner. </li><li>Identifies risk factors including lethality for suicidal/homicidal ideations and/or grave disability. As appropriate, complete involuntary mental health holds (27-65) according to Center protocol and applicable legal statutes. </li><li>Participates in Evidence Based Practices (including CBT, DBT, and other trauma treatments) as well as other agency initiatives (such as CAMS, a modality designed to identify and reduce drivers for suicidality). </li><li>Meets a target number of consumer service hours (direct hours with client contact). </li><li>Adheres to documentation completion and corporate compliance guidelines. For example, 90% of progress notes are to be completed within three business days, and 95% of CCARS and treatment plans are to be completed by the due date (annually and bi-annually, respectively). Discharges a majority of disengaged clients within sixty (60) days of no contact. </li><li>Participates in staff development activities to enhance professional growth, including attending mandatory in-services and training/educational programs designed to enhance knowledge about the impact of trauma and recovery thereof. </li><li>Participates in supervision by coming prepared with an agenda. Reports high risk/complex cases and utilizes a problem-solving approach as well as feedback. Attends supervision at times and interval agreed upon with supervisor (weekly for pre-licensed candidates; at minimum monthly for licensed candidates after training period has elapsed). </li><li>Exhibits enthusiasm, courtesy, adaptability, flexibility, and spirit of cooperation in the work environment. Maintains effective interpersonal relations with consumers, colleagues, management, and the public. Uses language and behavior to promote dignity and respect in line with Jefferson Center's mission statement. </li><li>Utilizes a trauma-informed lens to understand how trauma impacts the lives of the people being served, so that every interaction is consistent with the recovery process and reduces the possibility of re-traumatization. </li><li>Completes assessments, and other initial engagement strategies as needed based upon needs of the team. This includes evaluating for appropriate diagnosis, within the scope of one's expertise, and referring to other programs within the Center as appropriate. </li><li>Respond to referrals via email within twenty-four hours, and outreach within twenty-four hours once referral is accepted. Process internal referrals submitted via Avatar in regular intervals. </li><li>Work with other ETC clinician(s) to assign referrals for the team and ensure all referrals have been connected appropriately. </li><li>Provide timely follow-up with consumers discharging from the hospital, in accordance with protocols (example: clients must be offered an appointment within 7 days of discharge). </li><li>Timely follow up with new consumers, largely those with higher risk, awaiting 1st therapy session. Assess needs and refer to appropriate resources. </li><li>Provide time limited case management to clients as indicated. </li><li>Assess and treat/refer to appropriate SUD-focused services, as indicated by needs of the client, and within the scope of clinician's practice (i.e., CAT, CAS, or LAC). </li><li>Work with other members of the ETC team to staff Open Clinic, an on-demand therapy model that takes place twice weekly. </li></ul> <p>Note: Employees are held accountable for all duties of this job.
