Make a living while you make a difference at Trilogy Health Services - a senior living provider with the continuous goal of being the Best Healthcare Company in The Midwest.</p> <p>POSITION OVERVIEW</p> <p>Oversees the coordination of resident services, documentation, onboarding, and engagement activities for the Assisted Living community. Serves as a key liaison among residents, families, staff, and external partners to ensure smooth communication and high-quality service delivery. Ensures that services support the comfort, satisfaction, and overall experience of residents.</p> <ul> <li>Collaborates with campus leadership to support the day-to-day coordination of resident services and overall operational flow. </li><li>Ensures that pre-admission information is collected and communicated to appropriate team members and monitors for non-clinical changes that require follow-up. </li><li>Oversees Lifestyle and Care Needs information gathering to support accurate service planning, without performing clinical assessments. </li><li>Supports residents with non-clinical daily living needs such as dining assistance, social engagement, and general comfort. </li><li>Ensures CareTracker and other documentation systems are updated accurately and promptly by staff. </li><li>Reviews documentation for completeness and communicates needed updates to appropriate team members. </li><li>Leads and assists in developing service plans in collaboration with residents, families, and interdisciplinary team members, ensuring clear communication to caregivers. </li><li>Facilitates Assisted Living Resident Meetings and ensures timely follow-up on feedback and concerns. </li><li>Coordinates non-clinical aspects of pharmacy/medication processes such as communication, documentation flow, and compliance tracking, without handling or administering medications. </li><li>Serves as a liaison between residents, families, external providers, and campus teams to support communication and service coordination. </li><li>Supports staff in following established service routines and ensures adherence to company policies and state guidelines. </li><li>Assists in training caregivers on non-clinical topics such as documentation, customer service, infection-prevention practices, and resident engagement. </li><li>Participates in interviewing and selecting caregivers and support staff for the Assisted Living unit. </li><li>Assists leadership in recruitment, onboarding, and ongoing training of non-clinical team members. </li><li>Ensures employees meet required CPR/First Aid and orientation training standards per state and company policy. </li><li>Participates in weekend management rotation and provides on-call support for non-clinical issues as needed. </li><li>Supports marketing efforts by participating in tours, inquiries, and move-in coordination when appropriate. </li><li>Executes strategies to build relationships with referral sources and distributes campus information as needed. </li><li>Serves on and participates in campus committees such as customer service, quality assurance, and policy review. </li><li>Attends required company in-service training programs. </li><li>Other duties as assigned. </li></ul> <p>Education: High School / GED</p> <p>Experience: 0-1 years</p> <p>Licenses and Certifications</p> <ul> <li>Valid CPR certification </li><li>Valid drivers' license and ability to operate a large van/bus (up to 15-passenger) vehicle </li></ul> <p>Sitting, standing, bending, reaching, stretching, stooping, walking, and moving intermittently during working hours. Must be able to lift at least 50lbs.
