We are seeking a Case Management Coordinator to join our team.</p> <p>Job Summary: The Case Management Coordinator confers with students, parents, school personnel, and community agencies regarding problems or concerns related to student attendance and welfare through home visits, parent conferences, and school-based meetings including classroom presentations.</p> <p>Duties and Responsibilities:</p> <ul> <li>Provides assistance to schools helping with individual attendance and disengagement problems. </li><li>Maintains continuous communication with a variety of public agencies providing services to youth and parents. </li><li>Works with parents and students to improve school attendance and disengagement (knows and understands attendance laws, regulations, policies and procedures, as well as the connection between attendance and school achievement). </li><li>Informs school personnel on problems of attendance, disengagement, and general child welfare (attendance monitoring including chronic excused and/or unexcused absences). </li><li>Provides direct services for intervention, and coordinates community resources for students and families with the goal of improving attendance. </li><li>Completes reports for the purpose of evaluating intervention attempts for truancy and chronic absence. </li><li>Fosters communication for the purpose of establishing positive relationships between parents, students, staff and others. </li><li>Makes home visits with parents and students for the purpose of checking on chronically absent or truant students and gaining parental support in students' educational and social development. </li><li>Connects students and families to wrap around services. </li></ul> <p>Qualifications - To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.</p> <p>Education and Experience:</p> <ul> <li>BS, BA, or AA or AS degree or working towards degree in related field. </li><li>Ability to work effectively with students, parents, teachers, management staff, and community agencies; ability to adapt work schedule (work some evenings and/or weekends). </li><li>Experience: Preferred successful experience in a public-school setting preferred (working with students, parents, school staff, and community agencies). </li><li>Any combination of training and/or experience demonstrating the required skills, knowledge, and abilities may be considered by the District Administration and/or the Governing Board. </li></ul> <p>Other Skills and Abilities:</p> <ul> <li>Ability to maintain confidentiality of student, staff, or sensitive information is non-negotiable. </li><li>Ability to establish and maintain effective working relationships with students, peers, parents and the community. </li><li>Ability to speak clearly and concisely in written and oral communications. </li><li>Ability to perform duties with awareness of district requirements and BOE policies. </li><li>Must have excellent organizational and time management skills, verbal (including listening) and non-verbal communication skills. </li><li>Must be able to provide demonstrated leadership in the execution of communication strategies. </li><li>Must be able to work independently. </li><li>Pushing the agenda and goals of the school district is imperative. </li><li>Political acumen and organizational support is strong focus as well. </li></ul> <p>Benefits:</p> <p>Southwest Local School District participates in the Southwest Ohio Organization for School Health (SWOOSH).
