It's real.</p> <p>RN - Night shift - 7pm-7am - 36 hours a week with every other weekend. Long term care experience required. Regular schedule. Benefits along with a generous 25% weekend differential and 2$/hr night differential and .50 eve differential. Comfortable work load with a supportive team who love what they do!</p> <p>POSITION SUMMARY:</p> <p>Provides nursing care to residents using the principles of the nursing process from assessment through evaluation. Follows the CT Nurse Practice Act and follows McLean Nursing Philosophy of Excellence, Leadership, Integration, Learning and Discovery, while administering and directing ancillary personnel in the performance of nursing tasks. Takes responsibility for the delivery of care on the unit and engages in proper hand-off to the next shift.</p> <p>JOB FUNCTIONS: (This list may not include all job duties assigned.)</p> <p>Arrives for duty on time and ready to assume responsibility for the unit when assigned. Organizes resident/patient care according to accepted routines and prioritizes activities, and manages work day so as to complete duties within eight hour shift.</p> <p>Responsible and will be held accountable for ongoing assessments of the residents' nursing care needs. Immediately reports changes in condition of residents with accurate assessments and interventions to supervision, APRN or physician.</p> <ul> <li>Transcribes orders from physician in timely manner of consultant/or post physician visit. </li><li>Assists the MDS Coordinator in the development of the plan of care for each resident/patient. </li><li>Provides nursing care to residents in accordance with standard nursing practice. This care will include the administration of medications and patient treatments as prescribed by the physician. Documents care and resident response to medications and nursing interventions. </li><li>Assists in the provision of patient/family teaching and education. Documents patient understanding in the medical record. This includes both formal and informal teaching sessions. Examples of teaching include prescribed medications, activities of daily living, treatments and procedures. </li><li>Takes and assesses vital signs as indicated by the plan of care or change in patient condition. </li><li>Responsible to wear care unit pager at all times and respond when signaled, except during lunch period. </li><li>Responsible to read and follow PCC messaging. </li><li>Knows and is able to carry out approved emergency procedures. </li><li>Responsible for and prepares As & Is and follows-up to completion. </li><li>Monitors IV therapy, documents, and discontinues IV therapy. </li><li>Follows Standard Precautions and ensures procedures are followed by ancillary staff. </li><li>Participates in and/or provides information for resident planning conferences. </li><li>Communicates resident information to other members of the healthcare team and to resident's family. </li><li>Maintains a safe environment for residents, staff, and visitors and reports any concerns to supervisor. </li><li>Assigns resident assignments for ancillary staff. Reviews and monitors patient care performed by ancillary personnel. Collects and documents quality care indicators. Provides feedback to supervision. </li></ul> <p>Promotes the mission and values of McLean, and exhibits professional manner at all times in both speech and body language. Supports department and unit initiatives.</p> <p>Manages time appropriately, completes duties within 8-hour shift and/or requests supervisory assistance when necessary to ensure compliance, prior to end of shift.</p> <p>Participates in continuous learning opportunities both professionally and personally. Notifies supervisor or nurse educator on learning needs to perform nursing care.</p> <ul> <li>Abides by McLean Nursing Philosophy, policies and procedures of the Nursing department. </li><li>Performs other activities as assigned. </li></ul> <p>EMPLOYMENT REQUIREMENTS:</p> <p>Knowledge of nursing theory and practice. Knowledge and competency in the nursing process, quality assurance, infection control, hospice philosophy, restorative, rehabilitative, and gerontological nursing practice and theory. Able to interpret written instructions, communicate and document effectively. Ability to assess, plan, implement, evaluate, and document care and instructions. Ability to prioritize appropriately and make sound nursing judgments. Demonstrates personal integrity and empathy and fosters such in others.</p> <p>Graduate of an accredited Registered Nurse program.</p> <p>Current Connecticut State License in good standing as a Registered Nurse.</p> <p>Minimum of one-year experience in LTC, working with geriatric population.</p>
