Together, we create our culture by living our values in our day-to-day interactions with our patients and teammates.</p> <p>Job Summary</p> <p>Implements policies and procedures, along with standards of care and practice related to patient care needs involving various types of wounds. Provides direct patient care assistance to staff nurses and acts as consultative service regarding wound care to clinical staff. Performs wound care functions under the oversight of a Wound Care Coordinator.</p> <p>Essential Functions</p> <ul> <li>Provides care for patients whose primary needs involve wounds. </li><li>Evaluates all wounds upon admission and ongoing to determine treatment plan and provide early problem identification. </li><li>Reviews initial treatment plan with the supervising Registered Nurse and Wound Care Coordinator. </li><li>Reports to the supervising Registered Nurse and Wound Care Coordinator any change of wound status. </li><li>Assists in planning patient's care for treatment of wounds and prevention of wounds. </li><li>Evaluates products and cost effectiveness for use in hospital and makes recommendation. </li><li>Reviews and participates in developing discharge plans. </li><li>Educates staff on products available for use in hospital. </li><li>Evaluates culture results, wound progression, and collaborates with physician and Wound Care Coordinator on treatment plan. </li><li>Assists as needed with completion of root cause analysis and CMS quality data reporting. </li></ul> <p>Knowledge/Skills/Abilities/Expectations</p> <ul> <li>Ability to adapt to new situations, set priorities, and utilize problem-solving techniques. </li><li>Knowledge of wound care. </li><li>Knowledge in wound debridement as indicated within level and scope of practice. </li><li>Ability to serve as resources to nursing staff in complex wound management. </li><li>Ability to lead, motivate, and develop others individually and as a team. </li><li>Ability to work cooperatively as a member of a team. </li><li>Approximate percent of time required to travel: 0% </li><li>Must read, write and speak fluent English. </li><li>Must have good and regular attendance. </li><li>Performs other related duties as assigned. </li></ul> <p>Qualifications</p> <p>Education</p> <ul> <li>Graduate of an accredited nursing program leading to licensure as an LPN/LVN or RN. </li></ul> <p>Licenses/Certifications</p> <ul> <li>Current state RN or LPN/LVN license. </li><li>BLS certification. </li></ul> <p>Experience</p> <ul> <li>Three (3) years nursing experience with experience as an acute care nurse. </li></ul>
