<p>At JEVS, we help the people we serve lead self-determined, connected, and hopeful lives that strengthen themselves, their families, and their communities. We believe everyone deserves fulfilling jobs, educational opportunities, and access to vital services.</p> <p>The Wellness Manager will assess operations, lead annual plan, enables leadership at all levels. Provides direction to Director of Community Support Services, Associate Directors and Nurses related to wellness issues for the individuals. Provide ongoing training and education to individuals and employees as needed. Creates and develops opportunities within the seven dimensions of wellness; an annual plan will be created to sustain improvement and create new opportunities.</p> <p>Job Overview:</p> <p>To advance the activities and to facilitate wellness as the primary focus of everyday living. Quality of life is defined by the Individual but to be enabled by being well. Advancing wellness within the JEVS community substantially advances quality of life.</p> <ol> <li>Specific Nutrition </li></ol> <p>Provide access to knowledge about healthy eating and cooking; Develop tools and strategies to support re-learning eating and menu planning; Assess Intervention needed to advance health thru nutrition; Develop opportunities for S & R to show off" their efforts.</p> <p>2.Spiritual</p> <p>Provide connections to/from consumers and staff and community spiritual/religious center; Provide for pastoral support for Hassel residents; Heighten awareness of the religion in a healthy life; Assure access to pastoral support related to loss and grieving.</p> <p>3.Physical Activity</p> <p>Heighten the awareness of exercise as a part of a healthy lifestyle; Provide access to resources; Reconnect w/SO for those who desire the competition option; Develop goals to challenge people to get moving.</p> <ol start="4"> <li>Social Leisure </li></ol> <p>Provide assessments of Interest and time management; Provide access to knowledge of community centers, clubs, organization supports, hobbies, music, dance, art; Renew social and leisure choices and set goals to advance.</p> <ol start="5"> <li>Psychological </li></ol> <p>Provide support to evaluate outcome of mental health supports; Maintain knowledge of MIT system - access, services available; Represent JEVS at MH collaborative efforts.</p> <ol start="6"> <li>Health </li></ol> <p>Provide assistance to locate health resources; Assess trends in health status which may inspire wellness actions; Serve as JEVS rep on health related collaborative and effort.</p> <ol start="7"> <li>Relationships </li></ol> <p>Assess whether consumers have or are making Mends and connections with "non-staff"; Assist staff to encourage involvement of families as desired by the consumer; Make connections with other organization who support friend making.</p> <p>Scope of Responsibility:</p> <ul> <li> <p>Ongoing communication with Physician, Directors, Wellness team and residential staff to coordinate the healthcare needs of individuals, while maintaining continuity and consistency of care.</p> </li><li> <p>Act as a liaison between physicians, pharmacists, medical supply companies, etc. as needed.</p> </li><li> <p>Ongoing traveling to various Community Living Homes to monitor, implement, and coordinate the care needs of the individuals.</p> </li><li> <p>Review on a weekly basis and document on those targeted individuals who require close supervision by medical personnel.</p> </li><li> <p>Provide quality assurance reviews of medication and healthcare systems</p> </li><li> <p>Create Health Care Plans for all medically involved individuals.</p> </li><li> <p>Review all medical history summaries for accuracy and content.</p> </li><li> <p>Review medical records and complete medical concern sheets quarterly to ensure accurate documentation and to intervene in any necessary medical concerns.</p> </li><li> <p>Assess, coordinate and attend hospital discharge planning needs with team members.</p> </li><li> <p>Document and coordinate health care services in accordance with appropriate state regulations and nursing standards.</p> </li><li> <p>Implement physician orders throughout group homes.</p> </li><li> <p>Provide ongoing training and education to individuals and Direct Support Professionals.</p> </li><li> <p>Assign tasks and evaluate work for direct reports. Provide direction and feedback.</p> </li><li> <p>Prepare documentation for 90-day Psychiatric Medication Reviews; and attend when necessary.</p> </li><li> <p>Complete nursing assessments, nursing audits quarterly for all residents</p> </li><li> <p>Ensure Safe medication Systems.</p> </li><li> <p>Train and Coach DSP's at site on health-related issues</p> </li><li> <p>HRST Clinical reviews, updates and maintain communications with Associate Directors and Program Specialist for completions.</p> </li><li> <p>Assure nursing notes are completed in Carelogic</p> </li><li> <p>Collaborate with families, providers, and medical teams to ensure quality service.</p> </li><li> <p>Attend ISP meetings, as needed.</p> </li><li> <p>On-call responsibilities for medical issues/concerns.</p> </li><li> <p>Additional responsibilities as needed.</p> </li><li> <p>Valid PA Nursing license; BSN or RN Required.</p> </li><li> <p>Excellent communication skills and the ability to work with a team. Ability to problem solve.</p> </li><li> <p>At least five year's experience in the human services field.</p> </li><li> <p>Valid driver's license required.</p> </li></ul> <p>The base pay for this position ranges from $80,000 to $88,000 and will vary based on factors such as skills and experience. Base pay is one part of the Total Compensation that JEVS Human Services provides to recognize employees for their work.
