<p>Position Overview</p> <p>Under general supervision is responsible for providing case management and/or treatment according to established policies and procedures. The individual will assist in creating and fostering an environment conducive to the treatment, operational and administrative goals and mission of MonDay.</p> <p>Summary of Job Duties</p> <p>Essential Duties and Responsibilities</p> <p>Case Management</p> <ul> <li>Sets appointments, completes transportation requests, visitor lists, furlough verifications and obtains court orders </li><li>Determines if residents have their social security card, birth certificate and state ID/driver license </li><li>Assists with obtaining above mentioned documentation as needed </li><li>Investigates and set up court dates for residents with warrants </li><li>Assists in the facilitation with classes to support when clinical staff are out of the office </li><li>Provides transportation as necessary </li><li>Organizes and document notes according to standards </li><li>Assists staff in treatment and aftercare planning including appropriate linkages and progress of the offender while in the program in meeting target behaviors, goals and recommendations </li><li>Provides case management services by coordinating with other agencies, resources, and professionals within a planned framework of action toward the achievement of established goals </li><li>Responds to crisis situations and uses appropriate crisis intervention techniques </li><li>Follows the standardized process to periodically and objectively assess resident progress in meeting case plan goals through established defined criteria </li></ul> <p>Related Programming</p> <ul> <li>Interfaces with staff and residents </li><li>Assists the clinical staff with case management concerns </li><li>Documents all case management information in the progress and other client related data </li></ul> <p>Operations</p> <ul> <li>Follows and enforces Behavior Management System for managing residents </li><li>Ensures that necessary documentation is completed </li><li>Assists in monitoring residents and transportation </li><li>Performs other operational duties as needed </li></ul> <p>Administrative</p> <ul> <li>Participates in weekly team meetings, treatment team meetings, and all staff meetings </li><li>Participates in the development of MonDay policy and procedures </li><li>Performs public relations with all personnel and the community </li><li>Obtains annual training as required </li><li>Performs other duties as required </li></ul> <p>Quality Assurance</p> <ul> <li>Audits resident records for currency, completeness, and accuracy </li></ul> <p>Working Conditions</p> <ul> <li>Potential exists of exposure to disease/illness. </li><li>This is a minimum security facility, therefore all outside access doors are locked at all times. </li></ul> <p>Physical Requirements</p> <p>The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.</p> <ul> <li>Ability to lift, pull, push, and manipulate equipment. </li><li>Ability to sit and/or stand for long periods of time. </li><li>Ability to operate an automobile. </li></ul> <p>All the above duties and responsibilities are essential job functions subject to reasonable accommodation. Employees may be required to perform any other job-related instructions as requested by their supervisor, subject to reasonable accommodation.</p> <p>Direct Reports</p> <p>This position has no supervisory duties.</p> <p>Minimum Qualifications and Requirements</p> <p>Qualifications</p> <ul> <li>High School diploma or GED with 5 years' experience and current Chemical Dependency Counselor Assistant or LSW Assistant required. </li><li>Associate's degree preferred in a behavioral science in counseling, social work, or a related field and able to obtain a Chemical Dependency Counselor Assistant or LSW Assistant within ninety (90) days of appointment to position. </li><li>Must have 40 hours of Chemical Dependency specific education </li><li>Must maintain certification requirements of the Ohio Chemical Dependency Board or the State of Ohio Counselor and Social Work Board, as appropriate </li><li>Valid State of Ohio Driver's License with a good driving record required </li><li>Skills require the ability to analyze, plan, and implement case management concerns which may include coaching, counseling, and positive reinforcement </li><li>Ability to read and write routine reports, statistical information </li><li>Interpersonal communication skills requiring ordinary courtesy and tact are necessary to communicate effectively with staff, clients, families, social services, and criminal justice agencies, as well as the general public. </li><li>Ability to complete and follow through with case management concerns </li><li>Ability to facilitate groups </li><li>A working knowledge of the general principles of social work, sociology, psychology, criminal justice, or associated curriculum </li><li>Familiarity with Cognitive Behavior and Motivational Interviewing and a working knowledge of support group programs </li><li>Familiarity with standard office software operating systems and programs such as Microsoft Office, Microsoft Word and Excel. </li></ul> <p>Supplemental Information</p> <p>Background Check Information</p> <p>Criminal Background Check and BMV Check</p>
