<p>NURSE I</p> <p>GENERAL SUMMARY</p> <p>The purpose of this job is to provide comprehensive assessment and evaluation of needs for the older adult population to maintain quality of life while aging in the community. Develops care services, including monitoring, supporting, and advocating the specific needs of vulnerable adults for access to services; engages in the research of opportunities for community resources, bridging clients' needs with the local community.</p> <p>This job provides nursing care in the community setting. Develops, implements, and monitors a variety of public health programs, based on division and role. Conducts outreach in the clinical and community setting. Performs assessments, completes referrals, and maintains documentation. Performs care coordination, case management, and public education. Specific duties will vary based on the assigned division.</p> <p>ESSENTIAL DUTIES & RESPONSIBILITIES</p> <ul> <li>Performs related work as required. </li><li>Participates in pandemic response, may include participating in mass vaccination clinics. </li><li>Provides 24/7 response to any Emergency/Bioterrorism events. </li></ul> <p>Area Agency on Aging (AAA)</p> <ul> <li>Conducts telephonic and in-home comprehensive client assessments to determine psychosocial, financial, and environmental strengths and limitations and medical status/conditions of individuals aged 60 and over. </li><li>Performs case management duties such as assessment of clients (telephonic and in-home), monitoring contacts with clients, person-centered service plan adjustments with clients, communication with vendors, and coordinating services with clients and vendors, and maintains appropriate documentation in accordance with Division standards. </li><li>Establishes and maintains ongoing relationships with Aging Network organizations, including but limited to hospital discharge planners, human service providers, home care agencies, long-term care facilities, and the Michigan Department of Health and Human Services. </li><li></li></ul> <p>MINIMUM QUALIFICATIONS -</p> <p>Required Education and Experience</p> <ul> <li>At least two years of relevant college-level course work or an associate's degree from an accredited college or university in Nursing </li><li>1 to 3 years of related work experience in nursing </li><li>Any equivalent combination of training, education, and experience that provides the required skills, knowledge, and abilities </li></ul> <p>Required Licenses or Certifications</p> <ul> <li>Registered Nurse in the State of Michigan </li></ul> <p>VETERANS are encouraged to apply: Please provide your joint services transcript with your application.</p> <p>Pay Scale-109 X</p> <p>Monday-Friday 8:00 a.m.-5:00 p.m.</p> <p>Position : 10981007</p> <p>Code : 25120-1</p> <p>Type : INTERNAL & EXTERNAL</p> <p>Job Family : PUBLIC HEALTH & WELFARE</p> <p>Posting Start : 08/25/2025</p> <p>Posting End : 12/31/9999</p> <p>Details : Benefits Information</p> <p>MINIMUM SALARY: $69,076.80</p>
