<p>Job Description Summary</p> <p>The Nurse Navigator position will provide guidance and support to dementia patients and their caregivers within the newly formed South Carolina Alzheimer's Network (SCAN). SCAN is a multi-site clinical program at MUSC Health designed to provide patients with cognitive disorders an early and accurate diagnosis and treatment plan, longitudinal co-management with primary care, access to the new Alzheimer's disease medications, and access to the latest research.</p> <p>This full-time, on-site position reports directly to the Lead Dementia Care Navigator. SCAN is a multi-site clinical program at MUSC Health designed to provide patients with cognitive disorders an early and accurate diagnosis and treatment plan, longitudinal co-management with primary care, access to the new Alzheimer's disease medications, and access to the latest research.</p> <p>This full-time, on-site position reports directly to the Lead Dementia Care Navigator. This position actively supports SCAN patients and their families in navigating the complexities of the dementia related diagnosis, including the treatment process, available resources, education, crisis prevention, and care coordination.</p> <p>Dementia Care Nurse Navigator Job Duties:</p> <ul> <li>Clinical Evaluation 35%: </li><li>Assess the health status of patients through history taking, identify normal, and abnormal findings. </li><li>Help develop individualized care plans based on assessed needs, including medical management, behavioral support strategies, and community resource utilization. </li><li>Implement communication strategies for persons with dementia and their caregivers. </li><li>Facilitate referrals by obtaining clinical information and ensure the information is properly processed though corresponding departments. </li><li>Care Coordination and Monitoring 30%: </li><li>Review upcoming appointments in Epic, including those for Telehealth clinics, to ensure necessary services are arranged before the appointment. </li><li>Ensure all testing and referrals are complete and necessary items addressed with the provider. </li><li>Evaluate all patient test results, such as imaging and lab work, with SCAN providers and execute the care plans discussed. </li><li>Coordinate with referring physicians' offices to meet the needs of both patients and physicians. </li><li>Provide comprehensive education on dementia symptoms, disease progression, treatment options, coping strategies, and caregiver support to patients and families, resulting in increased understanding and improved outcomes. </li><li>Communication and Collaboration 35%: </li><li>Serve as primary clinical contact for SCAN patients and their caregivers regarding scheduling of testing and appointments. </li><li>Serve as the liaison between the SCAN clinical team and referring physicians. </li><li>Provide emotional support and counseling tailored to the patient's clinical situation. </li><li>Communicate with external case management and social services to ensure patients have access to individualized community support services. </li></ul> <p>This is a fulltime position that will be performed during normal business hours Monday-Friday on location. Flexibility in hours and location (e.g., work from home) to accommodate the candidates schedule may be possible if they do not interfere with SCAN operations.</p> <p>Core Requirements:</p> <ul> <li>Registered Nurse with a current and valid license. </li><li>Significant experience working with Alzheimer's disease patients and their families. </li><li>Fluent in English </li><li>Proficient in use of EPIC and Microsoft Office preferred </li><li>Knowledge of CPT codes, medical billing, and diagnosis coding helpful </li><li>Experience in medical practice and knowledge of medical terminology is required. </li></ul> <p>Additional Job Description</p> <p>Bachelor's degree in Nursing from an accredited school of nursing and a minimum of three years related clinical nursing experience required. Master of Nursing (MSN) or Doctor of Nursing Practice (DNP) in lieu of BSN. Excellent written and verbal communications skills as well as mentoring/coaching and organizational skills required. Demonstrated ability to develop, implement and evaluate effective patient and staff educational programs. Creative and resourceful in problem solving and exceptional critical thinking required. Licensure as a registered nurse by the South Carolina Board of Nursing or a compact state.
