<p>JOB CONTENT</p> <p>Position Summary:</p> <p>(Overall purpose of this position)</p> <ul> <li>Fulfill the medication needs of patients in a direct care outpatient setting through the processing and accurate dispensing of prescription orders, patient consultation and advisement, and by following state and federal laws and regulations as well as company policy and procedures. </li></ul> <p>Essential duties and responsibilities which must be performed in order to carry out the position purpose summarized above:</p> <p>(The below stated duties and responsibilities are intended to outline those functions typically performed by the incumbent in this position. This description of duties is not intended to be all-inclusive or to limit the discretionary authority of supervisors to assign additional tasks of a similar nature or level of responsibility.)</p> <ul> <li> <p>Evaluate prescriptions are written for a legal and legitimate purpose, process and accurately dispense prescriptions in accordance with state and federal laws and regulations and company policy and procedures.</p> </li><li> <p>Model, implement, and offer guidance and supervision to pharmacy technicians on customer service approaches and practices to ensure all patient needs, concerns, and other issues are addressed in a timely manner in accordance with NFP policies and procedures.</p> </li><li> <p>Maintain accurate pharmacy documentation in accordance with state and federal laws and regulations.</p> </li><li> <p>Medication home delivery process.</p> </li><li> <p>Provide comprehensive patient care by:</p> </li><li> <p>Applying medical knowledge in the advisement of patients and their medication needs.</p> </li><li> <p>Informing patients on proper medication practices and maintenance.</p> </li><li> <p>Educate patients in chronic disease self-management, as applicable.</p> </li><li> <p>Performing medication reconciliation evaluations on patients.</p> </li><li> <p>Collaborating with providers on the best treatment plan for patients.</p> </li><li> <p>Utilizing electronic medical records to assist in the advisement of patient care.</p> </li><li> <p>Work as a care team member to support NFP as a Patient Centered Medical Home (PCMH).</p> </li><li> <p>Prescribe or advise prescribing as permitted by license, state pharmacy board, and care agreement.</p> </li><li> <p>Assist provider and care team in population health activities and monitoring or chronic diseases.</p> </li><li> <p>Effectively precept students, interns and residents. Maintain and/or perform any necessary preceptor development training.</p> </li></ul>
