In each case the Treatment Coordinator will assure that the services delivered both to the target child/adolescent and to the family are individualized, appropriate to need, child centered, family focused, culturally competent, strengths based, and goal driven.</p> <p>Responsibilities</p> <ul> <li>Assess and identify the needs of the birth family to achieve their family service plan goals. </li><li>Empower birth families through systems education and advocacy training. </li><li>Involve and coordinator regular contact with biological family in all aspects of treatment planning including regular updates on youth's progress. </li><li>Work in collaboration with other service providers and community resources to develop and implement intervention procedures in the homes, working toward achievement of the permanency plan. </li><li>Represent agency regarding court involvement of the youth. </li><li>Prepare written summaries and directly supervise all activities including; Individual Service Plans and quarterly updates, discharge summaries, regular home visits, respite services, phone calls, and program contract regarding youth and families. </li><li>Assist and support in training activities for TFC parents, including providing in-home training as needed for TFC parents. </li><li>Complete a formal TFC Parent Evaluation including creation and monitoring of a professional development plan for each family. </li><li>Provide 24-hour emergency "on-call" service to TFC families according to designated schedule, responding to all complaints/concerns. </li><li>Actively participate in individual and/or group supervision, staff meetings, and all other professional trainings. </li><li>Provide ongoing advocacy and support for the child's educational needs. </li><li>Assure that all policies, procedures and standards relating to child safety and well-being are followed. </li></ul> <p>Qualifications</p> <ul> <li> <p>Education Qualifications:</p> </li><li> <p>A doctorate or master's degree in social work from a college or university accredited by the Council on Social Work Education or a field related to social work such as sociology, psychology, special education, or counseling, with a student placement in casework services to children and families or one year of experience in providing casework services to children and families;</p> </li><li> <p>A baccalaureate degree in social work or a field related to social work including, but not limited to, sociology, psychology, special education, or counseling and one year of experience in providing casework services to children and families; or</p> </li><li> <p>A baccalaureate degree in any field plus two years experience in providing casework services to children and families.</p> </li><li> <p>Clearances: State Police; FBI clearance; child abuse clearance; any additional background checks/clearances required by state governing bodies.</p> </li><li> <p>Valid driver's license and current vehicle insurance.</p> </li></ul> <p>Working Conditions</p> <ul> <li>Physical Demands: Requires vision, speech, and hearing. </li><li>Environmental Factors: Community, home, school. </li><li>Working Hours: A non-traditional work schedule as defined by service needs and crisis "on-call". </li></ul>
