<p>Hospice Licensed Practical Nurse (LPN)</p> <p>Position Overview</p> <p>We are seeking a compassionate and dedicated Hospice Licensed Practical Nurse (LPN) to join our team in New Berlin, WI. The ideal candidate will provide high-quality nursing care to patients in hospice, ensuring comfort and dignity during end-of-life care. This role requires strong clinical skills, empathy, and the ability to work collaboratively with interdisciplinary teams. This position requires on-call responsibilities also.</p> <p>Key Responsibilities</p> <ul> <li>Provide skilled nursing care to hospice patients in accordance with established care plans and physician orders. </li><li>Monitor patient condition and report changes to the interdisciplinary team promptly. </li><li>Administer medications and treatments as prescribed, ensuring patient safety and comfort. </li><li>Assist with pain management and symptom control to enhance patient quality of life. </li><li>Educate patients and families on disease progression, care techniques, and available resources. </li><li>Document nursing care and patient responses accurately and timely in compliance with regulatory standards. </li><li>Collaborate with physicians, social workers, chaplains, and other team members to develop and update individualized care plans. </li><li>Provide emotional support to patients and their families during the hospice journey. </li></ul> <p>Qualifications</p> <ul> <li>Current Licensed Practical Nurse (LPN) license in the state of Wisconsin. </li><li>Previous experience in hospice or palliative care preferred. </li><li>Strong knowledge of hospice nursing principles and end-of-life care. </li><li>Excellent communication and interpersonal skills. </li><li>Ability to work independently and as part of a multidisciplinary team. </li><li>Compassionate, patient-centered approach to care. </li><li>Basic computer skills for documentation and communication. </li></ul> <p>Work Environment</p> <p>This position involves providing care in patients' homes, nursing facilities, or hospice centers within the New Berlin area.
