Join us in delivering exceptional care-every shift, every resident, every time.</p> <p>Summary of Responsibilities:</p> <p>The Personal Care Appointment Scheduler is a hybrid role that combines essential scheduling duties with direct personal care responsibilities with a variable daily workload based on resident needs. In this role, the Scheduler provides high-quality direct and indirect care, ensuring the physical, emotional, and psychosocial well-being of residents. A key responsibility is proactive care coordination, including timely scheduling of medical and wellness appointments, arranging transportation, and facilitating in-home or external support services. The Scheduler also works collaboratively with families, healthcare providers, and community partners to maintain each resident's dignity, independence, and overall quality of life. The Personal Care Appointment Scheduler is expected to support all areas within Personal Care and demonstrate flexibility in balancing care and administrative tasks, contributing to a positive, person-centered environment for all residents.</p> <p>The content of this job description is intended to describe the general nature and level of work expectations performed by incumbents in this classification. All activities are completed in accord with the Core Focus, 10-Year Target, Core Values of the Company, following all policies, procedures, Equal Employment Opportunity laws, confidentiality, and other Federal, State and Local laws and regulations, as appropriate.</p> <p>Essential Functions/Duties:</p> <p>Scheduler Duties</p> <ul> <li>Coordinate and manage all aspects of medical appointment scheduling, including liaising with healthcare providers and family members to ensure appropriate follow-up and continuity of care. </li><li>Secure and arrange transportation for residents to medical appointments or personal errands as needed, ensuring timeliness and resident comfort. </li><li>Facilitate and coordinate in-home care services in collaboration with Home Health, Hospice, or private-duty providers to ensure resident safety and well-being. </li><li>Maintain documentation related to appointments, referrals, transportation logistics, and home care services in Point Click Care and other relevant platforms. </li></ul> <p>Personal Care Assistant Duties</p> <ul> <li>Assists with activities of daily living, (ADL's and IADL's) including assisting with transfers, mobility, dressing, personal hygiene including but not limited to mouth care, foot care, showering, episodes of incontinence, shaving, medication storage and distribution, ambulation, transportation coordination and companionship, light housekeeping, and the dining experience. Monitors Oxygen and performs glucose monitoring. </li><li>Monitors and reports changes in resident conditions and reports to licensed nurse or administrator. </li><li>Assist residents with medications and treatments prescribed for self-administration under the direction of a licensed nurse. </li><li>Communicates effectively with Families, PCP's and Pharmacy's as needed. </li><li>Serves meals and provides cues and assistance as needed to promote an engaged dining experience. </li><li>Performs light housekeeping duties in resident apartments and household common areas, team rooms, including but not limited to trash collection, linen changes and bed making, light vacuuming, dusting, and spot cleaning as needed. </li><li>Actively participates and engages in the Hearthstone Philosophy by following the daily flow. </li><li>Implements programming with residents (small group setting), including but not limited to cooking, baking, crafts, music and social activities. </li><li>Accurate documents in Point Click Care. </li><li>Maintains open lines of communication during shift and at shift change. </li><li>Learns life stories and engages residents based on likes and dislikes. </li><li>Encourages resident's independence in performance of all daily living activities as appropriate. </li><li>Understands special needs of the cognitively impaired resident and utilizes appropriate interventions and approaches </li><li>Observes and reports changes in condition and/or behaviors to supervisor/DOW promptly. </li><li>Is flexible and adapts to changing situations. </li><li>Follows THAW Policies and Procedures </li><li>Practices appropriate infection control and safety awareness. </li><li>Follows and supports practices to keep residents safe and free from harm, actively participates in improving resident safety. </li><li>Reports unsafe equipment and hazards to supervisor promptly. </li><li>Has knowledge of DHS regulations as it pertains to daily assignments. </li><li>Participate in and complete all required ongoing training, including monthly online and in-person training, annual skills competencies education, and role-specific certifications, to ensure continued competence and adherence to organizational and regulatory standards. </li></ul> <p>Other Duties:</p> <ul> <li>As assigned by management </li><li>Essential functions are not intended to be an exhaustive list of all position duties required and performed by the incumbent in the role. Work during inclement weather is required.
