Document the request, contact information, physician response, patient follow up and any other pertinent information in the EMR.</p> </li><li> <p>Review patient data for relevant clinical updates, laboratory, x-ray and other test results.</p> </li><li> <p>Implement and positively reinforces physician/provider orders.</p> </li><li> <p>Receive and respond to telephone calls in response to acute/chronic illnesses. Obtain chief complaint and/or history of present illness. Utilize MD approved guidelines based on patient complaint and make appropriate disposition of patient following Deaconess protocol. Document all data obtained in the Electronic Medical Record (EMR).</p> </li><li> <p>Medication list, pharmacy preference, demographics, allergies and any change to medical/surgical history should be reviewed and updated during each patient telephone encounter.</p> </li><li> <p>Confers with the patient and physician regarding scheduling priorities for acute and routine visits and when necessary, directing the patient to a higher level of care.</p> </li><li> <p>Utilizes the EMR to relay scheduling information for all patient referrals, diagnostic services, tests, procedures, acute/follow up appointments as directed by physician.</p> </li><li> <p>In collaboration with the physician, provide the patient and family with appropriate education regarding medications, treatments, tests, procedures, results and dietary recommendations. Per HIPAA Guidelines.
