<p>The Opportunity: We are seeking to hire a Nurse Care Coordinator to join our Regional Case Management Team. This individual works closely with the behavioral health provider, primary care providers, behavioral health team members, and the community agencies; provides professional assessment, coordination/planning of multiple health care services, and an ongoing evaluation of care management services to ensure that necessary clinical services are received. The Nurse Care Coordinator will process consults and deliver nursing care based on the client's competency and cognitive need, and facilitate/re-enforce education while fostering client accountability and self-management skills.</p> <p>About Us: For over 35 years, Community Support Services has been making a difference in the lives of countless individuals with severe and persistent mental illnesses.
