We are teachers, researchers, innovators, leaders and, most of all, caregivers.</p> <p>Some highlights of this opportunity include:</p> <ul> <li>Participate in the decision-making process in relation to admission to hospice at home or in the inpatient setting </li><li>Participating as a member of the Interdisciplinary Team in the development of individualized care plans </li><li>Certify (re-certify when necessary) patients eligible for hospice care under the Medicare Hospice Benefit </li><li>Consult with attending physicians regarding pain and symptom management </li><li>Coordinate efforts with the attending physician to provide continuity of care </li><li>Develop and monitor the medical component of the hospice program </li><li>Participate in the development, coordination and implementation of hospice services and policies </li><li>Participating in the development of staff and volunteer training </li><li>Conduct home visits (in person and virtual) as a consulting physician when appropriate </li><li>Act as a hospice liaison and advocate with the national, state and local physicians, healthcare professionals, partners and community </li><li>Perform related administrative functions as required and/or assigned </li><li>Part time - 8 hours per week </li></ul> <p>Located between Boston and New York City, Connecticut offers you and your family access to a lifestyle that is second to none.
