As a Case Manager II at Seven Hills Rhode Island, you will play a vital role in empowering participants to make informed decisions, access essential resources, and achieve their goals through person-centered planning. This conflict-free position ensures participants receive comprehensive, unbiased support to thrive in their chosen environments.</p> <p>Responsibilities</p> <ul> <li>Facilitate and support participants in creating individualized, person-centered plans. </li><li>Guide participants in understanding and exercising their right to make informed choices, including self-direction. </li><li>Assist with Medicaid renewals and other eligibility processes to ensure uninterrupted services. </li><li>Provide timely, effective communication, and respond promptly to participant needs. </li><li>Ensure participants are aware of their rights, including the right to be free from abuse, neglect, and exploitation. </li><li>Monitor and reassess participant plans to align with evolving needs and preferences. </li><li>Collaborate with participants, their support networks, and service providers to coordinate effective, non-duplicative services. </li></ul> <p>Qualifications</p> <ul> <li>Education: Bachelor's degree required. </li><li>Experience: 3-5 years of related experience. </li><li>Skills: Strong communication, time management, assessment, and person-centered planning skills. </li><li>Knowledge: Understanding of community resources, rights and dignity, and person-centered services. </li><li>Other Requirements: Valid driver's license and ability to drive. </li></ul> <p>Why Join Seven Hills Rhode Island?At SHRI, we value diversity, collaboration, and innovation.
