<p>Employment Type:</p> <p>Full time</p> <p>Shift:</p> <p>Description:</p> <p>Position Summary:</p> <p>The Nurse Navigator is a key member of the multidisciplinary team functioning as an advocate, educator and facilitator of care. Responsible for ensuring all referred patients receive quality and comprehensive services throughout the continuum. Coordinates the overall subspecialty programmatic goals and outcomes.</p> <p>What you will need:</p> <ul> <li>Bachelor's degree in Nursing </li><li>Preferred: Master Degree </li><li>Current State of Michigan Registered Nursing License </li><li>BLS active certification </li><li>Three years of experience in an acute care setting. </li><li>One year of experience in medical, surgical or radiation oncology </li><li>Preferred: Case management experience or patient care leadership experience </li><li>Ability to lift up to a maximum of 35lbs. and perform push/pull tasks that require a maximum force of 35 lbs. Ability to physically operate safe patient movement equipment in compliance with the Safe Patient Movement Policy. </li></ul> <p>What you will do:</p> <ul> <li>Completes a comprehensive health assessment of the patient's physical, psychological, social, environmental, financial and functional status in collaboration with multidisciplinary team members. Includes assessment of past and present treatment course </li><li>Assists in organizing and coordinating timely patient appointments </li><li>Understands and develops goals, treatment plan and provider options in concordance with evidence based standards and clinical trials </li><li>Works directly with physician lead to review diagnostic studies, clinical history and course of treatment to identify most appropriate next course of care </li><li>Collaborates with multidisciplinary team members in performance improvement activities including assessment, planning, implementation and evaluation of clinical outcomes and processes for patients </li></ul> <p>What you will work:</p> <ul> <li>Full time </li><li>40 hours per week </li><li>Day Shift </li></ul> <p>Our Commitment</p> <p>Rooted in our Mission and Core Values, we honor the dignity of every person and recognize the unique perspectives, experiences, and talents each colleague brings.
