<p>Employment Type:</p> <p>Full time</p> <p>Shift:</p> <p>Description:</p> <p>Position Summary:</p> <p>The Nurse Navigator is a key member of the multidisciplinary team functioning as an advocate, educator and facilitator of care. Responsible for ensuring all referred patients receive quality and comprehensive services throughout the continuum. Coordinates the overall subspecialty programmatic goals and outcomes.</p> <p>What you will need:</p> <ul> <li>Bachelor's degree in Nursing </li><li>Preferred: Master Degree </li><li>Current State of Michigan Registered Nursing License </li><li>BLS active certification </li><li>Three years of experience in an acute care setting. </li><li>One year of experience in medical, surgical or radiation oncology </li><li>Preferred: Case management experience or patient care leadership experience </li><li>Ability to lift up to a maximum of 35lbs. and perform push/pull tasks that require a maximum force of 35 lbs. Ability to physically operate safe patient movement equipment in compliance with the Safe Patient Movement Policy. </li></ul> <p>What you will do:</p> <ul> <li>Completes a comprehensive health assessment of the patient's physical, psychological, social, environmental, financial and functional status in collaboration with multidisciplinary team members.
