We believe that high-performing teams drive groundbreaking medical discoveries and invite all applicants to join us and experience what it means to be part of Mass General Brigham.</p> <p>Clinical Duties:</p> <ul> <li>Identify patients in the Emergency Department who may be candidates for Palliative Care consultation: emergent inpatient consultation, inpatient consultations within the first 24-48hrs of hospital admission, and/or outpatient/home based involvement </li><li>Huddles with Emergency Department, Palliative Care, and other clinical teams to assess patient appropriateness for program protocols and pathways. </li><li>Educate clinicians and staff on programmatic pathway criteria and facilitate appropriate patient entry into the corresponding pathway. </li><li>Follows patient progression into appropriate pathway and through to placement into the next appropriate step in their care. </li><li>Interface with Clinical Team and Case Management regarding patient disposition. </li><li>Gathers pertinent clinical and social information to identify appropriate level of care and location of care within the hospital/health system. </li><li>Ensures patients are assigned to appropriate Palliative Care service lines. </li><li>Maintains data and other medical records for the NWH ED-embedded Palliative Care Program </li><li>Participates in regular case review, data analysis, and quality improvement efforts. </li></ul> <p>Job Summary</p> <p>The registered nurse renders highly professional and technical nursing care to assigned patients. The registered nurse provides direct and indirect patient care using the nursing process (assessment, planning, implementation, and evaluation).
