<p>This Hospice/Palliative Care Coordinator RN position is in Geriatrics and Extended Care service at the Charlie Norwood VA Medical Center Uptown, located at the Augusta, GA . This position will be responsible and accountable for all elements of the nursing process when providing and/or supervising direct care to terminally ill and end of life Veteran population.</p> <p>Duties include, but are not limited to:</p> <ul> <li>Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. </li><li>Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. </li><li>Administers medications and procedures per established policies and guidelines. </li><li>Influences care outcomes by collaborating with members of the interdisciplinary team. Core elements of performance include knowledge and active participation in the unit/program level quality improvement process and initiatives as well as customer service programs. </li><li>Compassionate End-of-Life Care: Provide compassionate and dignified care to patients in their final journey. Offer support and comfort to both patients and their families during a challenging time. </li><li>Meaningful Connections: Create deep and meaningful connections with patients, fostering trust and understanding.
