<p>Maternal Edu Specialist, Maternal One Model A Brief Overview Responsible for assessing the health behaviors and health literacy needs of program clients to promote wellness throughout the perinatal period. Serves as high-risk OB clinical expert and resource, providing knowledge, skills and training for clients with complex prenatal health conditions and/or co-morbidities including but not limited to diabetes, sickle cell disease, HIV, and hypertension. What you will do Coordinates and manages evidence-based client education strategies that support optimal health and wellness throughout client#s perinatal journey and beyond. In collaboration with the physician and other health care professionals, determines and develops educational interventions that best suit the physical, cognitive, emotional and health literacy needs of program clients, based on individualized assessments. Guides clients in making personal goals in order to improve their health and better manage their perinatal journey. Utilizes clinical judgment and critical thinking skills in assessing and analyzing patient data, as well as, evaluating patient responses to education. Identifies, collects, and reports input and output metrics/data on education program goals and effectiveness including solutions for process improvement and efficiencies in high-risk perinatal prevention strategies and training. Actively participates in program and multidisciplinary client rounds to report updates/ outcomes on individualized education care plans. In collaboration with program manager and community outreach coordinator, develops, coordinates and participates in marketing strategies to promote healthy perinatal lifestyles and healthcare engagement. Collaborates with perinatal social workers, vendors, healthcare staff, and community resources in identifying at-risk individuals and connecting them to skill-building activities that promote healthy perinatal lifestyles and/or support resources. Advocates for improved health resources, policies, procedures, and services that promote the health and mental resiliency of program clients. Conducts needs assessment surveys, timely reviews and solicits advise on novel methods to successfully provide integrated solutions, such as multimodal education and training plans, meaningful electronic documentation and efficient processes toward improving healthy perinatal lifestyle behaviors. Ensures the education program produces the specified results in the monthly/ annual work plan(s) to the required quality standard and within the timeline and budget parameters of the grant. # Ensures the program#s client education plans are in compliance with regulatory and organizational policies. # Coordinates with the childbirth educator to promote prenatal education classes for our clients. # Reports on client educational engagement, referrals, well-being initiatives, and results that demonstrate improvement in program metrics. Participates in outreach, patient recruitment, and community events as determined by program needs. Engages in professional development activities to maintain and enhance expertise. Qualifications Associate#s Degree Associate#s Degree in Nursing Required Bachelor#s Degree Bachelor#s Degree in Nursing Preferred Professional licensure in applicable discipline required (i.e. Registered Nurse license, Respiratory Therapist license, etc.) Required Minimum 3 years experience OB clinical experience in hospital setting Required high-risk perinatal patient education background Preferred Proficiency in navigating and documenting in Electronic Medical Record, experience with Cerner platform.
