<p>Hours of Work: Week 1: M-Th, Sat & Week 2: Sun, Tu-Fri 8:30am-5:00pm</p> <p>Provides person-centered behavioral health assessment, care, resources and education to clients in need of crisis services. Provides support for emergency rooms, law enforcement and other community agencies. Provides treatment planning, short term treatment, and linkage to ongoing care for clients across the lifespan; Pursue the least restrictive disposition that will meet the determined safety needs of clients. Prioritize needs of the community and demands on the crisis unit; Review and oversee development and implementation of safety plans. Documents care provided. Collaborates with internal and external colleagues. Participates in clinical and administrative meetings. Supports and promotes the Mission, Vision and Guiding Principles of the Human Services Department. Performs in a manner consistent with the HSD Professional Standards and "Commitment to My Co-Workers."</p> <p>50%- Provides person-centered behavioral health care services for clients experiencing a behavioral health crisis</p> <p> 1. Assess clients in crisis to determine least restrictive disposition that will assure client safety</p> <p> 2. Assess for voluntary admission options for individuals without a funding source.</p> <p> 3. Authorize chapter 51.15 emergency detentions when appropriate. </p> <p> 4. Coordinate admissions, when clinically indicated, to the appropriate </p> <p> receiving facility.</p> <p> 5. Coordinate medical clearance if required with police, referring facility, and hospital ER </p> <p> staff.</p> <p> 6. Work with individuals and natural supports to create appropriate safety plans when</p> <p> clinically indicated and in consultation with the crisis supervisor.</p> <p> 7. Develop therapeutic relationships with clients</p> <p> 8. Create person-centered crisis response plans, crisis plans and updates</p> <p> 9. Provide individual, family, and/or group brief psychotherapy </p> <p> 10. Coordinate client care and discharge planning as needed</p> <p> 11. Provide linkage to psychiatry and other services or resources as indicated.</p> <p> 12. Provide clinical consultation to HSD and community partners as needed to support service coordination</p> <p>30%- Consult and assist with recommendations to law enforcement agencies about those </p> <p>individuals who are experiencing an AODA related crisis.</p> <p> 1. Assess for and facilitate detox admission under 51.45</p> <p> 2. Provide pre-admission and post-discharge suicidal assessments for detox clients.</p> <p> 3. Coordinate referrals to county AODA specialist. </p> <p> 4. Collaborate with security or psych tech staff for transport to and from detox facilities.</p> <p> 5. Document recommendation for treatment by detox providers. </p> <p>10%- Conduct correctional detox, suicidal, and homicidal assessments.</p> <p> 1. Provide crisis services at the Rock County Jail </p> <p> 2. Provide crisis intervention services at the Youth services Center </p> <p> 3. Coordinate the admission process when a detox or psychiatric hospitalization is clinically</p> <p> indicated.</p> <p> 4. Collaborate with Sheriff Office and YSC staff for crisis planning to maintain safety and</p> <p> inmate/youth behavioral management. </p> <p> 5. Provide back up work for court related services when needed</p> <p>5%- Ensure program and medical record compliance </p> <ol> <li> <p> Follow all departmental as well as divisional policies and procedures </p> </li><li> <p>Provides unit oversight to scheduled welfare checks and safety plan process </p> </li><li> <p>Document client care in accordance with policies, procedures, and regulatory guidelines </p> </li></ol> <p>3%- Participates in all clinical and administrative activities as required</p> <ol> <li> <p> Participate in case review meetings </p> </li><li> <p>Participates in crisis planning meetings</p> </li><li> <p> Participate in staff meetings</p> </li><li> <p> Participate in meetings with supervisor as needed</p> </li></ol> <p>2%- Will perform other duties as assigned</p> <p> </p> <ul> <li>Knowledge of the principles and practices of behavioral health care and its application, including person-centered crisis plans, trauma-informed care, co-occurring disorders, and stages of change. </li><li>Knowledge of laws, regulations, and practices pertaining to federal and state human service and behavioral health care programs </li><li>Knowledge of assessment and treatment of suicide, mental illness and substance use disorders </li><li>Knowledge of psychotropic medications, their main effects, side effects, and abuse potential. </li><li>Knowledge of psycho-social rehabilitation services </li><li>Ability to maintain accurate records and prepare clear and concise reports </li><li>Ability to establish and maintain effective working relationships within the agency and community. </li><li>Ability to understand and carry out verbal and written instructions </li><li>Ability to take initiative, work independently, and seek out supervision as needed </li><li>Ability to manage one's time and emotions in an environment of rapidly changing priorities </li><li>Excellent computer skills related to electronic record keeping, email, and internet </li></ul> <p> </p> <p> Masters degree in behavioral health field required; </p> <p>LCSW, LMFT, or LPC preferred, or ability to obtain license within 3 years;</p> <p>Substance abuse credential preferred or ability and willingness to obtain certification;</p> <p>Experience working with clients with complex functional and mental health impairments;</p> <p>Must possess and maintain a current unrestricted (not due to corrective lenses) driver's license and auto insurance pursuant to the Rock County Administrative Policy and Procedure 5.02 and 5.27. </p> <p>Physical elements:</p> <ul> <li>Ability to sit for long periods throughout the day </li><li>Some physical activity related to retrieving and carrying records </li></ul> <p> </p> <p>Equipment use:</p> <ul> <li>Extensive computer including significant typing for documentation </li><li>Use of telephone, copier, and fax machine </li></ul> <p> </p> <p>Working conditions:</p> <ul> <li>Work is generally conducted in an office setting, while occasional community and home visits are likely </li></ul> <p> </p> <p>Note: Potential exposure to physically or verbally abusive clients </p>
