<p>Accura HealthCare is growing! Join as a Hospital Liaison as an integral part of our Marketing and Business Development team. This role involves facilitating patient transitions from hospital settings to our care environment, ensuring smooth communication, and enhancing the relationship between healthcare providers. The ideal candidate will have a strong background in healthcare, excellent interpersonal skills, and a deep understanding of the needs of both hospital patients and nursing home residents.</p> <p>This position requires frequent travel between nursing home facilities and various hospitals within the assigned region. The role may require occasional evening or weekend work to meet with hospital staff or attend community events.</p> <p>KEY RESPONSIBILITIES:</p> <ul> <li>Collaborate with hospital discharge planners, case managers, and social workers to identify patients who would benefit from our nursing home services. </li><li>Facilitate the transfer process, ensuring all necessary medical records and patient information are accurately and promptly shared between facilities. </li><li>Serve as the main point of contact for hospital staff regarding patient care transitions, updates, and concerns. </li><li>Conduct regular meetings with hospital staff to discuss potential patient cases, care plans, and follow-up care needs. </li><li>Develop and maintain strong, professional relationships with key hospital personnel to ensure ongoing cooperation and referral opportunities. </li><li>Represent our nursing home at community health events, hospitals, and medical conferences to promote our services. </li><li>Ensure all patient transfers comply with HIPAA regulations and other healthcare laws. </li><li>Maintain detailed records of patient interactions, transfers, and outcomes for reporting purposes. </li><li>Monitor the quality of the transfer process to enhance patient satisfaction and care continuity. </li><li>Address any issues or feedback from hospitals regarding the care transition process. </li><li>Provide training or informational sessions to hospital staff about our services, capabilities, and care, as needed. </li></ul> <p>REQUIRED QUALIFICATIONS:</p> <ul> <li>Minimum of 3 years of experience in a healthcare setting, ideally in patient care coordination or case management. </li><li>Previous experience working in or with skilled nursing or long term care facilties is advantageous. </li><li>Excellent communication and negotiation skills. </li><li>Strong organizational skills with attention to detail. </li><li>Proficiency in electronic health record systems and general computer literacy. </li><li>Ability to work independently and handle sensitive information with discretion. </li><li>Valid RN license or related healthcare certification is highly desirable. </li><li>Valid unrestricted Driver's License, personal transportation may be required. </li></ul>
