<p>As part of the Belmont Behavioral Health System, the Philadelphia Children's Crisis Response Center (PCCRC) is a program designed to help young people who are experiencing acute mental or behavioral healthcare struggles between the ages of 3 through 17.</p> <p>The Philadelphia Children's Crisis Response Center (PCCRC) is seeking full-time Social Workers to join our team. We are currently hiring for the 10am-10pm and 12pm-12am shifts.</p> <p>The Social Worker will be responsible for planning, directing, defining and coordinating individualized and group therapeutic support and services to patients with behavioral problems, psychiatric disorders, post-traumatic stress disorder, substance use and physical and emotional abuse.</p> <p>In addition to this great opportunity, Belmont offers:</p> <ul> <li>401(k) </li><li>Great Insurance Plan </li><li>PTO Plan </li><li>Tuition Reimbursement </li><li>Conduct psychosocial assessment of patient and develop an effective treatment plan that applies therapeutic principles appropriately while ensuring patient safety at all times. </li><li>Through patient observation and patient/family interview obtaining information on the patient's needs, skills, interests and limitations for the development and implementation of the treatment plan. </li><li>Provide crisis intervention and assist families in understanding the implications and complexities of a life situation and its impact on lifestyle. </li><li>Participate in multidisciplinary treatment team meeting to discuss psychosocial assessment, treatment planning and ongoing discharge planning. </li><li>Identify, assess and coordinate patient cases and anticipate their needs related to illness, hospitalization, finances, education and home/community services. </li><li>Facilitate education, process groups, support groups and referrals as requested. </li><li>Conduct intakes and assist in coordinating placements within facility programs as required. </li><li>Develop and execute clinically relevant activities in coordination with activity professionals, as required. </li><li>Promote recovery, symptom reduction and the achievement of the highest level of social functioning. </li><li>Communicate or aid family members in understanding and supporting the patient. </li></ul> <p>OTHER FUNCTIONS:</p> <ul> <li>Perform other functions and tasks as assigned. </li></ul> <p>EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:</p> <ul> <li>Master's degree in Social Work from an accredited college or university required. </li><li>Three or more years' related experience working with comparative patient population required. </li></ul> <p>LICENSES/DESIGNATIONS/CERTIFICATIONS:</p> <ul> <li>State licensure, as required by the state in which the facility operates; which could include: Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), Licensed Social Worker (LSW). </li><li>CPR and de-escalation/restraint certification required (training available upon hire and offered by facility). </li><li>First aid may be required based on state or facility. </li></ul> <p>While this job description is intended to be an accurate reflection of the requirements of the job, management reserves the right to add or remove duties from particular jobs when circumstances</p> <p>(e.g. emergencies, changes in workload, rush jobs or technological developments) dictate.</p> <p>BEL01</p>
