This role is an integral part of the Palliative Care Team, offering emotional support, resource referrals, and guidance during complex medical decisions. This position documents Social Work assessment, family meetings, interactions, family dynamics, and other pertinent information in the medical record. The Social Worker assists patients with completing advance directives and/or aides in obtaining previously completed advance directives.
