<p>Full Time Hospice Registered Nurse Case Manager (RNCM)</p> <p>Castle Senior Living, Inc. Hospice is looking for a Full Time Hospice Registered Nurse Case Manager (RNCM) for our Hospice program. Coverage area is in Racine, South Milwaukee and surrounding areas.</p> <p>Position Overview:</p> <p>RNCM would be responsible for providing skilled and compassionate nursing care and support to patients and families where they call home. Implements thorough skilled nursing assessments to include physical, psychological, social, spiritual, and environmental statuses, identify patient goals and appropriate interventions, and create an individualized plan of care for each patient. Performs skilled nursing care in accordance with standards of practice and per Castle Hospice policies and procedures and the patient's plan of care. Continuously reviews the plan of care and revises the plan of care in accordance with the fluidity of patient and family needs. RNCM would coordinating schedule with the Hospice Director of Nursing, or designee, to ensure 24/7 patient care coverage. This position would involve on-call, which would decrease as census enables for continued staff growth.</p> <p>Qualifications:</p> <p>Must have current certification as a Registered Nurse with the state of Wisconsin. Minimum of one year medical/surgical, hospice, home health, or skilled nursing facility experience preferred. Must possess exceptional communication, problem solving skills, and have a compassionate bedside manner.
