<p>This position provides case management and service linkage for individuals in need of ongoing behavioral health and substance use services. Services are provided via phone, in office and in the community based on the need of the person requesting service. This position maintains knowledge of admission criteria for Rock County Human Services programs, community behavioral health providers and other community resources. Supports and promotes the Mission, Vision, and Guiding Principles of the Human Services Department. Performs in a manner consistent with the HSD Professional Standards and "Commitment to My Co-Workers."</p> <p>75%- Provide support and linkage services for individuals in need of ongoing behavioral health and substance use disorder services.</p> <ol> <li> <p>Works closely with the crisis intervention team to triage phone calls from individuals who have non-emergent behavioral health service needs.</p> </li><li> <p>Provide screening and assessment of individuals to determine level of care and service needs. Screening tools that may be utilized include: CSSRS, ASAM, Functional Screen, Comprehensive Screening tool, and Recovery Need Level.</p> </li><li> <p>Guide individuals in accessing care through Rock County Human Services and/or community providers, through insurance and/or other payer programs.</p> </li><li> <p>Provide psycho-education and case management to reduce barriers to care and support individuals as they link to long term services.</p> </li><li> <p>Provide coverage for other connections team staff as assigned.</p> </li><li> <p>Participate in data collection for the purposes of grant funding and program evaluation.</p> </li><li> <p>Participate in case review or team meetings as assigned.</p> </li></ol> <p>20%- Ensure program and medical record compliance</p> <ol> <li> <p>Follow all state, county and departmental policies and procedures</p> </li><li> <p>Document client care in accordance with policies, procedures, and regulatory guidelines</p> </li><li> <p>Attend program or departmental meetings or trainings as required.</p> </li><li> <p>Assist in data collection and keeping.</p> </li></ol> <p>5%- Other duties as assignedProfessional expertise in the following areas preferred. Training and ongoing professional development will be provided.</p> <p>Knowledge of the principles and practices of behavioral health care and co-occurring disorders, including assessment of risk and safety planning.</p> <p>Knowledge of laws, regulations, and practices pertaining to federal and state human service programs, specifically chapters 34, 51,75, 92, 94.</p> <p>Knowledge of human service and community resources and how to effectively utilize them.</p> <p>Knowledge of Medicaid, Medicare, HMO's, Managed Care organizations, and public assistance programs.</p> <p>Knowledge of psychotropic medications, their main effects, side effects, and abuse potential.</p> <p>Knowledge of psycho-social rehabilitation services</p> <p>Ability to plan and organize work effectively.</p> <p>Ability to maintain accurate, current records, and prepare clear and concise reports from them.</p> <p>Ability to establish and maintain effective working relationships within the agency and community.</p> <p>Ability to understand and carry out verbal and written instructions.</p> <p>Knowledge and application of best practices in cultural competency, person centered care, and Trauma Informed Care. - Bachelors Degree from an accredited college or university in human services field.</p> <ul> <li>Available to work a flexible schedule including work into the evenings when needed to meet client needs </li><li>Must possess and maintain a current unrestricted (not due to corrective lenses) driver's license and auto insurance pursuant to the Rock County Administrative Policy and Procedure 5.02 and 5.27. Physical elements: </li><li>Ability to stand, walk and sit. Physical movement occurs a majority of the day. </li><li>Ability to enter client homes without assistance, including the ability to navigate stairs. </li><li>Inside and outside work with exposure to weather conditions. </li><li>Potential for contact with unpredictable and aggressive persons. </li><li>Potential for interactions with animals including but not limited to household pets. </li><li>Must be willing to transport clients and be exposed to challenging and unpredictable environments. </li><li>Participate in disaster evacuations and fire drills when circumstances warrant. Maintain safety procedures. </li><li>Lifting and carrying up to 25lbs. on occasion. </li></ul> <p>Equipment use:</p> <ul> <li>Ability to use a computer and keyboard. </li><li>Must become efficient at using a computerized documentation within 2 months of hire including the use of Microsoft Office products, an electronic medical record system, and other web based systems. </li><li>Ability to use a copy and fax machine as well as a cellphone. </li></ul> <p>Working conditions:</p> <ul> <li>Able to work in multiple locations including the office, consumer's homes, and a variety of community locations. </li><li>Available to work a flexible schedule including working evenings when needed to meet client needs. </li><li>Ability to flex time when needed to coordinate consumer emergencies, care and recovery plan services. </li></ul>
