Our health center provides same-day primary care services to every member of the community, regardless of life situation or ability to pay. We also provide a wide array of behavioral health services. Our community Health Care Centers are located at 281 Main St. East Hartford, 40 Coventry St. Hartford and 828 Sullivan Ave., South Windsor.</p> <p>InterCommunity's Addiction Services offers a similarly wide range of substance use treatment and recovery-supportive services, with many sites in the Hartford area. The Community Health Navigator plays a vital role in connecting clients to essential health services. This position supports individuals by coordinating appointments, providing transportation, participating in community outreach, and promoting both physical and mental well-being. The ideal candidate is compassionate, organized, and knowledgeable in basic medical and behavioral health concepts.</p> <p>Essential Duties & Responsibilities:</p> <p>Client Support & Navigation</p> <ul> <li>Schedule and coordinate medical, behavioral health, and social service appointments. </li><li>Provide safe and timely transportation to and from appointments and community events. </li><li>Assist clients in understanding care plans and navigating healthcare systems. </li></ul> <p>Community Engagement</p> <ul> <li>Represent the organization at community events, health fairs, and outreach activities. </li><li>Build relationships with local partners and service providers to enhance client support. </li></ul> <p>Health Education & Advocacy</p> <ul> <li>Educate clients on the importance of preventive care, chronic disease management, and mental health awareness. </li><li>Advocate for client needs within the healthcare system and community services. </li></ul> <p>Documentation & Communication</p> <ul> <li>Maintain accurate records of client interactions, appointments, and services provided. </li><li>Communicate effectively with clinical teams to ensure continuity of care. </li><li>Collaborating with healthcare professionals to ensure efficient patient care </li></ul> <p>Additional Responsibilities:</p> <ul> <li>Maintaining cleanliness and organization in patient care areas </li><li>Maintain accurate and up-to-date documentation of patient assessments, care plans, progress notes, and services rendered. </li><li>Ensure compliance with regulatory requirements and healthcare policies and procedures. </li><li>Adhere to all federal and state regulations and guidelines. </li><li>Always maintain patient confidentiality. </li><li>Perform other duties as assigned </li></ul> <p>Schedule:</p> <p>Monday - Friday, 8:30 AM - 5:00 PM</p> <p>Requirements</p> <p>Qualifications:</p> <ul> <li>High school diploma or equivalent required; associate degree or healthcare certification preferred. </li><li>Valid driver's license and reliable transportation required. </li><li>Strong interpersonal, organizational, and communication skills. </li><li>Empathy and cultural sensitivity </li><li>Problem-solving and resourcefulness </li><li>Team collaboration and independence </li></ul> <p>Education &/OR Experience:</p> <ul> <li>Experience in community health, social services, or a healthcare setting. </li><li>Understanding physical and mental health conditions. </li><li>Commitment to health equity and community wellness </li><li>At least 2-3 years of experience working in a medical setting. </li><li>Bilingual English to Spanish preferred </li><li>All Agency Staff Employees MUST show proof of CPR Certification within 60 days of official start date. </li></ul> <p>Salary Description</p> <p>Starting Hourly Rate $21.25</p>
