As an Endocrinology Community Health Worker, you will work closely with the diabetes team and patients to ensure they receive the resources and support they need to manage their condition effectively.</p> <p>This flexible hybrid position offers daytime hours (approximately 10 hours per week) and allows you to split time between in-person and remote work.
