<p>The Mental Health Intensive Case Management Program Registered Nurse (RN) provides high-level, community-based care for veterans with severe mental illness (SMI) to prevent hospitalization and enhance community adjustment. Key duties include medication management, crisis intervention, psychiatric assessment, and in-home support, operating within an interdisciplinary team.</p> <p>The Mental Health Intensive Case Management Program Registered Nurse (RN) provides high-level, community-based care for veterans with severe mental illness (SMI) to prevent hospitalization and enhance community adjustment. Key duties include medication management, crisis intervention, psychiatric assessment, and in-home support, operating within an interdisciplinary team.</p> <p>Duties include but are not limited to;</p> <ul> <li>Executes position responsibilities that demonstrate leadership, experience, and creative approaches to managing complex patients and their care </li><li>Provides direct and indirect nursing care to Seriously Mentally Ill (SMI) Veterans in the home and outpatient setting to reduce inpatient episodes </li><li>Carries a caseload of SMI patients with a full range of psychiatric diagnoses and is responsible for medication education for all MHICM patients </li><li>Coordinates the teaching of medication compliance techniques, including medication delivery in the home and office setting. </li><li>Demonstrates the ability to communicate confidently with members of the healthcare team and demonstrate professional behaviors that promotes cooperation and teamwork. </li><li>Manage a caseload of patients with diverse psychiatric diagnoses, including SMI. </li><li>Provide medication education and promote adherence, including home and office visits. </li><li>Coordinate care with a focus on patient self-management and recovery. </li><li>Advocate for patients, families, and caregivers to improve outcomes. </li><li>Incorporate patient and family input into care delivery without bias. </li><li>Develop recovery-oriented treatment plans aimed at mental health stabilization. </li><li>Stay informed on current trends and share updates with the team. </li><li>Travel within a 50 mile radius as required for home visits. </li></ul> <p>VA offers a comprehensive total rewards package: VA Nurse Total Rewards</p> <p>Pay: Competitive salary, regular salary increases, potential for performance awards</p> <p>Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)</p> <p>Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA</p> <p>Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)</p> <p>Licensure: 1 full and unrestricted license from any US State or territory</p> <p>Work Schedule: Monday-Friday 0730-1600</p> <p>Telework: Ad Hoc Only</p> <p>Virtual: This is not a virtual position.</p> <p>Relocation/Recruitment Incentives: Not Authorized</p> <p>EDRP Authorized: This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program.
