Serve as a primary contact for residents, families, and referral sources-providing guidance and support throughout the transition process. Collaborate with the interdisciplinary team to ensure individualized care plans are in place from day one. Support the Director of Nursing with select leadership functions, including staff mentoring, quality assurance, and regulatory compliance.
