The goal of the program is to assist clients with a path to employment. The SNAP Case Manager is responsible for providing implementation, assessments, career pathways, and overall assessment of individual SNAP customers. The case manager will work with clients throughout the initial intake visit until the client is excited from the program. The case manager collaborates with customers on job readiness skills, career assessment and planning, as well as assisting with opportunities to complete GED or further their education. Assess and determines program eligibility for new referrals with Program Manager. Assist program referrals to complete necessary paperwork during orientation process and is responsible for interviewing, assessment, and placement of participants.</p> <p>ESSENTIAL DUTIES AND RESPONSIBILITIES</p> <ul> <li> <p>Maintain a caseload of clients throughout workforce development programming and process</p> </li><li> <p>Initiate and maintain employer contacts to provide pathways to employment.</p> </li><li> <p>Initiate and maintain connections with assistive organizations to address barriers to employment (legal assistance, housing assistance, etc.)</p> </li><li> <p>Provide periodic workshops to help improve client employability.</p> </li><li> <p>Assist with monthly placement assignments of eligible participants. Interviewing skills and knowledge of employment guidelines a plus. Knowledge of standard business practice and management is preferred.</p> </li><li> <p>Manage customers' caseload in program management, billing spreadsheets for all provided services</p> </li><li> <p>Oversee case management and direct service staff regarding policies and procedures</p> </li><li> <p>Served as first point of contact for all case management issues</p> </li><li> <p>Maintain organized files and case notes</p> </li><li> <p>Track and report outcomes and provide data updates on participants as directed.</p> </li><li> <p>Participate in onsite and off-site training and meetings, as needed.</p> </li><li> <p>Completed weekly chart audits and necessary follow-up to ensure documentation standards being met</p> </li><li> <p>Maintained documentation and records as required by state for each individual and each assessment</p> </li><li> <p>Planned organized and coordinated programs with agencies and worksites partners</p> </li><li> <p>Assumed leadership activities in program manager absence</p> </li><li> <p>Assumes other duties as assigned by management.</p> </li></ul> <p>HOURS OF WORK</p> <p>Monday- Friday: 8:30 am- 5:30 pm</p> <p>EDUCATION REQUIREMENTS</p> <p>Bachelor's Degree in related fields of social services, human resources, business, sociology or business equivalent with minimum one year of experience in working with workforce development in a for-profit setting.</p> <p>PHYSICAL DEMANDS: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. After hours or weekend events may be required. You will be informed of your work schedule by your manager, and you are expected to be flexible, as your work schedule may be subject to change based on departmental needs. Exact times for meals breaks may vary based on work schedules, shifts and departments. Overtime and weekend work may be required and will only be assigned and authorized by supervisory personnel.</p>
