<p>Description</p> <p>Become part of an inclusive organization with over 40,000 teammates, whose mission is to improve the health and well-being of the unique communities we serve.</p> <p>Summary:</p> <p>As a Readmission Program Coordinator (Registered Nurse), you will play a key role in our mission to ensure exceptional healthcare for the people we serve by analyzing, recording trends, and developing strategies aimed at reducing hospital readmissions. This role emphasizes proactive data collection and analysis to identify common causes of readmissions, suggesting alternative approaches to care, and supporting care teams by providing evidence-based recommendations. You will also assist in identifying opportunities for improvement in care protocols to enhance patient outcomes and prevent readmissions. Reports to Director of Quality. Performs other duties as assigned to fulfill the mission of the quality department.</p> <p>Responsibilities:</p> <p>Key Responsibilities:</p> <ul> <li>Collect and analyze patient data, including readmission statistics, to identify trends, patterns, and risk factors to drive improvement. </li><li>Provide feedback to clinical staff based on data analysis to help refine patient care strategies. </li><li>Work with hospital, UNC system and community partners to improve transitions from inpatient to outpatient safely and effectively </li><li>Input data into Get with the Guidelines (GWTG) Database for heart failure patients. </li></ul> <p>Other InformationOther information:</p> <ul> <li> <p>EDUCATION</p> </li><li> <p></p> </li><li>Bachelor's degree in nursing required (master's degree preferred). <p></p> </li><li> <p>EXPERIENCE</p> </li><li> <p></p> </li><li>Minimum of 3 years of clinical experience, with at least 1 year in an acute care hospital or similar setting. <p></p> </li><li> <p>LICENSURE/REGISTRATION/CERTIFICATION</p> </li><li> <p></p> </li><li>Valid and current NC nursing license. <p></p> </li><li> <p>OTHER SKILLS AND QUALIFICATIONS</p> </li><li> <p></p> </li><li>Demonstrated experience in analyzing patient care trends and providing data-driven care recommendations. <p></p> </li><li> <p>Strong analytical and problem-solving skills with the ability to interpret complex datasets and work independently.</p> </li><li> <p>Excellent communication and collaboration skills for working with multidisciplinary teams.</p> </li><li> <p>WORK ENVIRONMENT</p> </li><li> <p>Schedule 8:30am - 5:00PM, Monday-Friday with hospital recognized holidays off.</p> </li><li> <p>May be required to work alternate schedule or additional hours to accommodate needs of the department.</p> </li><li></li></ul> <p>Job DetailsLegal Employer: Lenoir HealthEntity: UNC Lenoir Health CareOrganization Unit: Quality And Patient SafetyWork Type: Full TimeStandard Hours Per Week: 40.00Pay offers are determined by experience and internal equityWork Assignment Type: OnsiteWork Schedule: Day JobLocation of Job: US:NC:KinstonExempt From Overtime: Exempt: Yes</p>
