<p>The Community Empowerment Case Manager at Pike CAC will work closely with the Community Empowerment Director, Community Health Worker, and department staff to provide specific, measurable, and individualized service to each person served as part of a multi-disciplinary team. This position will work with the director and department staff to form and strengthen internal and external relationships that will benefit the clients we serve.</p> <p>Functional Area: Service Delivery, Partnerships, Strategic Planning</p> <p>Estimated Effort: 80%</p> <p>Responsibilities, Deliverables, Outcomes and Expectations</p> <ul> <li>Develop and implement individualized care plans with measurable outcomes in collaboration with students, faculty, and guardian. </li><li>Create documents, maintain databases, and send letters, memos, and emails. </li><li>Assist individuals in person, via phone calls, virtually and by email providing knowledgeable and informed information and responses. </li><li>Provide ongoing case management, including regular follow-up, monitoring, and adjusting care plans as needed. </li><li>Provide case management services including, as minimum, assessment, referral, and monitoring/follow-up. </li><li>Maintain a courteous and professional demeanor with customers, staff, funders, community partners while promoting a positive reflection of CAC and the Community Empowerment Department. </li><li>Demonstrate an understanding of systems of care, entitlements and benefits, and cultural awareness. </li><li>Completion of intake, care plans, case notes, forms, and data entry. </li><li>Maintain required records and handle confidential information in a professional manner, including HIPAA compliance. </li><li>Ensure that all documentation needs for billing are completed within 48-hours of service delivery </li><li>Advocates for clients to ensure equal access to services. </li><li>Assists the Community Empowerment Director and Community Health Worker with special projects. </li><li>Attends meetings, conferences, and training related to the duties of this position, and disseminates information to appropriate individuals. </li><li>Facilitate referrals to medical, mental health, substance abuse, and social services as appropriate. </li><li>Collaborate with interdisciplinary teams to coordinate care, ensuring all aspects of the client's needs are addressed. </li><li>Monitor clients' progress towards achieving their health and social goals, adjusting services as necessary. </li><li>Provide information about prevention, education, and health promotion programs available in the community. </li><li>Maintain accurate, up-to-date case files and documentation in compliance with organizational and regulatory standards. </li><li>Track and report client outcomes, program performance, and service delivery metrics. </li><li>Serve as a liaison between students, families, school staff, and external service providers. </li><li>Communicate regularly with teachers, administrators, and counselors to ensure students are supported in all aspects of their school life. </li><li>Ensure timely and accurate documentation of client interactions, assessments, care plans, and referrals. </li></ul> <p>Functional Area: Agency Service and Other Responsibilities</p> <p>Estimated Effort: 20%</p> <p>Responsibilities, Deliverables, Outcomes and Expectations</p> <ul> <li>CAC Committee assignments, and other agency service duties as assigned. </li><li>Liaison between Community Empowerment Department staff and CAC programs/Community Partners and Providers </li><li>Other tasks and duties as assigned by supervisor. </li></ul> <p>Education:</p> <ul> <li>High School Diploma or equivalent </li></ul> <p>Job-Related Experience:</p> <ul> <li>Valid Ohio Driver's License, clean driving record, reliable transportation, insurable under agency policy. </li><li>Proficiency in Microsoft Office. </li><li>One (1) year experience providing case management services, including proficiency with Microsoft Office and use of office equipment. </li></ul> <p>People Management Experience:</p> <ul> <li>Friendly, professional and courteous demeanor. </li><li>Strong communication skills; ability to interact productively with senior staff, CAC department staff, community partners, and clients. </li></ul> <p>Licenses, Certifications, and Accreditations:</p> <ul> <li>Eligibility to obtain QMHS certification (Qualified Mental Health Specialist) in first 90 days of employment. </li></ul> <p>Preferred Qualifications</p> <ul> <li> <p>Preference given to individuals with Bachelors or Associates Degree in social work, counseling, social services, human services or related field.</p> </li><li> <p>Preference given to individuals with education and/or training in mental health and/or substance use disorder.</p> </li><li> <p>Preference given to individuals with experience serving a non-profit or healthcare entity.</p> </li><li> <p>Preference given to individuals who have experience with casework management and casework software.</p> </li><li> <p>Preference given to individuals with Community Health Worker certification.</p> </li></ul>
