In addition, this position will be eligible for additional benefits consistent with the role. The salary of the finalist selected for this role will be determined based on various factors, including but not limited to: scope of role, level of experience, education, accomplishments, internal equity, budget, and subject to Fair Market Value evaluation. The hiring range listed is a good faith determination of potential compensation at the time of this job advertisement and may be modified in the future.</p> <p>What you will be doing</p> <p>EDUCATION - Required</p> <p>Bachelor's or Doctor of Pharmacy (Pharm D) from an accredited institution</p> <p>CERTIFICATION - Required</p> <p>Valid license and registration issued by NY State Board of Pharmacy</p> <p>EXPERIENCE - Preferred</p> <p>1 Year in Healthcare</p> <p>PHYSICAL WORKING CONDITIONS</p> <ul> <li> <p>May involve standing for extended periods and occasional lifting of pharmaceutical supplies.</p> </li><li> <p>Requires precision and attention to detail in a fast-paced, high-pressure environment.</p> </li><li> <p>Shift work, weekends, and holiday hours will be required based on hospital needs.</p> </li></ul> <p>ENVIRONMENTAL WORKING CONDITIONS</p> <p>Indoor area with frequent exposure Located in an indoor area with frequent exposure to mild physical discomfort from dust, fumes, temperature, and noise. Examples: patient care providers and laboratory technicians.</p> <p>HAZARDS</p> <p>Category 2 - plus</p> <p>Tasks that involve no exposure to blood, body fluids, tissues, and other potentially infectious materials; but employment may require performing tasks that involve, or may involve, exposure to OSHA regulated substances as stated in MSDS.</p> <p>POSITION ACCOUNTABILITIES/COMPETENCIES</p> <ul> <li> <p>Reviews medication orders, prescriptions, medication profiles for proper dosing range, allergies, drug interactions, drug-food interactions, and contraindications. Ensures appropriateness of medication dispensing for all infant, pediatric, adolescent, adult and geriatric patients.</p> </li><li> <p>Ensures Prescribers and Nurses are contacted promptly to verify any order that requires an intervention, or non-formulary medication.</p> </li><li> <p>Patient's are appropriately contacted if the patient's own medications are required. Patients' own medications are promptly retrieved for screening and labeling. They are then profiled, verified, and delivered to the appropriate nursing unit and storage area.</p> </li><li> <p>Maintains and develops competence through required in-service and continuing educational programs.</p> </li><li> <p>Provides current and accurate drug information to hospital personnel and patients. Demonstrates thorough understanding of new medications, dosage requirements, indications, contraindications, pharmacology, pharmacokinetics, side effects, adverse reactions, precautions, etc.</p> </li><li> <p>Maintains state compliance with USP 795; USP 797 and USP 800 guidelines during compounding of sterile and non-sterile medications.</p> </li><li> <p>Adheres to departmental policies and procedures. Complies with regulatory mandates - TJC, Department of Health, NYS Board of Pharmacy, DEA and rules and regulations of Medical Staff.</p> </li><li> <p>Visually inspects all medication prior to dispensation to a patient or ADM (Automated Dispensing Machine) to assure the correct medication is dispensed and the medication is suitable for use (Expiration Date)</p> </li><li> <p>Responds and participates in Signal One codes. Prepares medications as requested by the physician. Provides drug and dosing information as needed.</p> </li><li> <p>Documents interventions and pharmacy consultations and collects data for pharmacist impact in peri-operative and post-surgical setting.</p> </li><li> <p>Performs job-related duties as assigned, such as nursing unit inspections and record keeping.</p> </li><li> <p>Reports for duty punctually.</p> </li><li> <p>Maintains satisfactory attendance record.
