<p>Type:Full TimeSalary/Pay Rate:$60,000-$65,000/yearPosted Date:06/25/2025</p> <p>Juvenile Institutional Services</p> <p>Position Summary</p> <p>The Juvenile Case Manager provides intensive case management and advocacy for youth in Somerset and Hunterdon Counties who participate in programs administered by Juvenile Institutional Services, and who are assigned to the caseload by the Coordinator of Juvenile Institutional Services.</p> <p>Essential Functions</p> <ul> <li>Provides intensive case management and advocacy for youth in Somerset and Hunterdon Counties who participate in programs administered by Juvenile Institutional Services, and who are assigned to the caseload by the Coordinator of Juvenile Institutional Services. </li><li>Conducts Risk Assessments when assigned to youth in the Firewatch program. </li><li>Coordinates individual or group community service projects when assigned to youth in the Advanced Station House Adjustment Program. </li><li>Monitors the Question and Answer (QAL) Line and responds to callers within 24 hours of receiving a message. </li><li>Provides case management for and support to any new division and/or departmental program initiatives. </li><li>Set up case file for new assignments, and maintain typed case notes on a daily basis </li><li>Contact other agencies currently/ previously involved in the case to obtain background information </li><li>Conduct comprehensive child/family assessments in a timely manner, following the intake protocol as defined in each program's Standard Operating Procedures. </li><li>Complete and submit AIF forms for Somerset County youth upon acceptance to the program </li><li>Link youth to community-based services including, but not limited to school, evaluations and/or counseling, employment, community service </li><li>Perform in-person visits on a weekly basis (at home, school, probation, or another mutually agreed upon location) for youth in the JJICM and ASHA programs </li><li>Coordinate transportation for clients to appointments, programs, and interviews, and provide transportation as needed </li><li>Coordinate and facilitate communication between family members and key players in the youth's life (court officials, care managers, mental health/substance abuse counselors, school personnel, etc.) </li><li>Provide 24 hour on-call support to families and clients in the event of emergencies </li><li>Submit weekly program summary reports </li><li>Complete Summary Reports to the Judge, prior to a youth's scheduled court appearance </li><li>Ensure that court-involved youth and the parent /guardian are aware of all scheduled hearings, though primary notification will still come through Family Case Management </li><li>Attend scheduled proceedings with court-involved youth on her case load </li><li>Attend Child Study Team meetings, probation reporting, counseling appointments, etc. </li><li>Participate in weekly supervision sessions with the program. </li><li>Update case plans in the file as needed </li><li>Provide a comprehensive Closeout Summary to either the court (for youth in Home Detention or Advanced House Arrest) or the referral source (for youth in the JJICM, Advanced Station House Adjustment, and Firewatch programs) </li><li>Represent Juvenile Institutional Services as needed at meetings of the Multi-Disciplinary Team, Juvenile Officers Association, Juvenile Detention Alternatives Initiatives Subcommittees, and New Jersey Division of Fire Safety </li><li>Participate in training/certification as a Youth Firesetter Intervention Specialist through the NJ Division of Fire Safety; and certification as a Firesetter Specific Mental Health Evaluator, if eligible </li><li>Monitor the Question and Answer (QAL) Line and respond to callers within 24 hours of receiving a message. </li><li>Participate in program design, development, and implementation as needed to address the changing needs of the community </li><li>Attend job related workshops, trainings and conferences as requested and approved by the Coordinator of Juvenile Institutional Services </li><li>File, fax, collate, and shred confidential client information as needed. </li><li>Provide vacation/leave coverage of other caseloads as needed </li></ul> <p>When working within the Home Detention or Advanced House Arrest Programs, the JCM will also:</p> <ul> <li>Perform a minimum of 2 home visits per week, Monday through Friday for youth in Home Detention </li><li>Provide on-call supervision Monday 8:30am through Friday 4:30pm, as well as on certain major holidays and/or weekends; and switch on-call assignment to and from the covering worker on weekends and holidays </li><li>Receive and process all schedule change requests from families and clients for assigned youth in the Home Detention and Advanced House Arrest programs, and notify on-call worker of any changes in schedule </li><li>Provide weekly participant verification to local police departments for youth in the Home Detention and Advanced House Arrest program </li><li>In cooperation and coordination with Richard Hall Community Health & Wellness Center, provide case management for inmates at the Somerset County Jail in need of inpatient substance abuse treatment. </li></ul> <p>Essential Requirements</p> <ul> <li>Master's degree in criminal justice, social work or relate field with a minimum of 2 years' experience performing case management in the criminal justice field, preferably with youth. </li><li>Valid NJ Driver's License </li></ul> <p>Preferred Requirements</p> <ul> <li>Prior experience working with at-risk youth and families </li><li>Prior experience with public speaking </li><li>Prior experience providing customer service </li><li>Bilingual (English/Spanish) a plus </li></ul>
