<p>POSITION SUMMARY</p> <p>The Family Support Provider (FSP) for Stronger Families, Stronger Futures provides support and education in the home environment to first time parents and their children (pre-natal to age 5) who are at risk for child abuse and neglect. The FSP works in collaboration with service providers to reduce abuse or neglect. Fathering Family Support Providers are men who provide the home visiting services to fathers and males who have a significant relationship with the child and are in a caregiving role.</p> <p>GENERAL DUTIES AND RESPONSIBILITIES</p> <ul> <li> <p>Provides home visits to assigned families where children have been identified through program screening criteria as being "at risk" for abuse and/or neglect.</p> </li><li> <p>Provides home visits to assigned families who are identified as needing services including but not limited to parenting and child development education, advocacy and support and assistance with accessing community resources.</p> </li><li> <p>Makes home visits based on the needs of families.</p> </li><li> <p>Provides families with information that includes but is not limited to child development, parenting, safety, immunizations and community resources.</p> </li><li> <p>Utilizes program-approved curriculum to improve parenting skills and increase the understanding of normal child development.</p> </li><li> <p>Develops strength-based service plans to address areas identified as challenges.</p> </li><li> <p>Assists client in seeking additional services from community resources as needed.</p> </li><li> <p>Maintains collaborative relationship with primary care provider and community services.</p> </li><li> <p>Accepts new assignments as determined by the Supervisor.</p> </li><li> <p>Actively participates in supervision.</p> </li><li> <p>Immediately notifies supervisory staff of situations in which children may be at risk (i.e., physical abuse, sexual abuse, chronic neglect, unsafe home, etc) and/or any other sentinel event.</p> </li><li> <p>Maintains accurate record of all client contact (and collateral work) including service hours, number of home and office visits, contact with collateral agencies, travel time, office time and training hours.</p> </li><li> <p>Completes client contact log, all required paperwork and statistics per agency standard and requirements.</p> </li><li> <p>Testifies in court as needed.</p> </li><li> <p>Makes referrals to appropriate community resources.</p> </li><li> <p>Maintains the confidentiality of the client records and information at all times.</p> </li><li> <p>Attends and actively participates in unit and program staff meetings.</p> </li><li> <p>Promotes agency's services, programs, image and philosophy.</p> </li><li> <p>FSP who meet the credentialing requirements for the senior home visitor position may, in addition to their regular duties, be assigned leadership responsibilities within the home visiting program.</p> </li><li> <p>PQI Functions</p> </li><li> <p>Ensure the consistent accurate collection of the data</p> </li><li> <p>Identify patterns and trends for program</p> </li><li> <p>Use results of data to inform supervisor of trends that may impact services</p> </li><li> <p>Performs other duties as assigned by the supervisor.</p> </li></ul> <p>QUALIFICATIONS:</p> <ul> <li>Bachelor's degree or Associates Degree working towards a Bachelor's degree preferably in a human service field with knowledge of Community Resources. </li><li>Family support providers should have experience working with vulnerable children and families. </li><li>Experience working with children and families in crisis preferred. </li><li>Knowledge of child abuse and neglect reporting criteria preferred. </li><li>Good communication skills - written and verbal. </li><li>Ability and willingness to work with families of all racial and socio-economic backgrounds. </li><li>Basic computer skills Word and Excel preferred. </li></ul> <p>REQUIREMENTS:</p> <ul> <li>Valid Connecticut State driver's license </li><li>Automobile liability insurance for $100,000-$300,000 </li><li>May require weekend and/or evening hours to attend functions or meet with families. </li></ul> <p>EMPLOYEE BENEFITS</p> <p>Catholic Charities offers access to a comprehensive array of benefits, including:</p> <p>For Good Health: Medical, Prescription, Dental and Vision insurance</p> <p>For a Secure Future: Life, AD&D and Long-term disability insurance</p> <p>For Retirement: 403(b) Plan, with employer matching contributions for eligible employees</p> <p>For Career Advancement: Training and development opportunities</p> <p>For Work Life Balance: Generous paid time off; including 14 paid holidays, vacation and sick time.</p> <p>Employee Assistance Program (EAP) - Free and confidential counseling for employee and immediate family</p> <p>Catholic Charities Inc., Archdiocese of Hartford is in compliance with all applicable discrimination laws.
