<p>SUMMARY:</p> <p>Reporting to the Pediatric Social Work Manager, the Sickle Cell Program Coordinator assists patients and families in resolving broad, medically related problems by locating and coordinating appropriate personal, hospital, community, and financial resources, within a specialty service area. The core Success Factors include:</p> <p>Instill Trust and Value Differences</p> <p>Patient and Community Focus and Collaborate</p> <p>PRINCIPAL DUTIES AND RESPONSIBILITIES:</p> <p>Consistently applies the corporate values of respect, honesty and fairness and the constant pursuit of excellence in improving the health status of the people of the region through the provision of customer-friendly, geographically accessible, and high-value services within the environment of a comprehensive integrated academic health system.</p> <p>Arranges and coordinates connective referrals to appropriate community agencies and programs as needed and ensures Unite Us is utilized effectively for this work.</p> <p>Assists with the planning and coordination of social work and community health work assessments, interventions, and supports. This will include attending meetings, making phone calls, training staff, and providing real time guidance to integrated behavioral health team.</p> <p>Collaborates with other support service providers in SC Clinic, and is responsible for ensuring efficient referrals and use of resources, as well as minimizing redundancy with patient/family supports.</p> <p>Is responsible for knowing and acting in accordance with the principles of the Lifespan Corporate Compliance Program and Code of Conduct.</p> <p>Interviews patients and families to obtain pertinent information concerning patient and family needs in the context of family functioning as needed.</p> <p>Reviews patient treatment plans and ensures appropriate services and referrals are made and executed.</p> <p>Maintains appropriate documentation and participates in Quality Improvement projects to meet expectations of clinic leadership.</p> <p>May participate in departmental studies and projects to include planning and coordinating activities. Will specifically assist with data collection and reporting efforts in SC Clinic.</p> <p>Assists with maintaining list of hospital and community services and resources and ensures all collaborators in SC Clinic have equal access to this information.</p> <p>Maintains and enhances professional expertise through appropriate educational activities as necessary.</p> <p>Assists in the resolution of practical problems of daily living and other concrete services, including transportation, housing, education, and training, etc.</p> <p>Reports directly to the Manager of Pediatric Social Work at Hasbro Children's Hospital.</p> <p>Perform other job duties as assigned.</p> <p>Performs other duties as assigned.</p> <p>EDUCATION:</p> <p>Master's degree in social work from a school accredited by the American Council on Social Work Education required. Current LICSW (or eligible for current licensure) from State of Rhode Island.</p> <p>EXPERIENCE:</p> <p>Experience working with patients diagnosed with sickle cell disease preferred.</p> <p>Demonstrated knowledge and skills necessary to provide care to patients with consideration of aging processes, human development stages and cultural patterns in each step of the care process.</p> <p>Five plus years of professional experience working in health care setting or human service agency preferred.</p> <p>Knowledge of health care and health care delivery systems.</p> <p>Analytical skills are necessary to evaluate patients' concrete needs and to formulate and implement a treatment plan, as well as assess how full team are completing these tasks to reduce redundancy and increase efficiency and effectiveness.</p> <p>Interpersonal skills to effectively interact with patients, families, medical staff, and outside agencies in providing services.</p> <p>SUPERVISORY RESPONSIBILITY:</p> <p>Supervises up to 3 integrated behavioral health staff.</p> <p>Pay Range:</p> <p>$69,732.94-$115,039.29</p> <p>EEO Statement:</p> <p>Brown University Health is committed to providing equal employment opportunities and maintaining a work environment free from all forms of unlawful discrimination and harassment.</p> <p>Location:</p> <p>Rhode Island Hospital - 593 Eddy Street Providence, Rhode Island 02903</p> <p>Work Type:</p> <p>M-F 8a-4:30p</p> <p>Work Shift:</p> <p>Day</p> <p>Daily Hours:</p> <p>8 hours</p> <p>Driving Required:</p> <p>No</p>
