The MDS Coordinator will coordinate the entire RAI process assuring the accuracy, timeliness, and completeness of the MDS, CAAs, and Interdisciplinary Care Plan.</p> <p>QUALIFICATIONS:</p> <ul> <li>Health - Ability to meet performance requirements. </li><li>Education, Experience and Skills: </li><li>Graduate from an approved school of nursing, College or University. </li><li>Must have a current, unencumbered license to practice nursing as a Registered Nurse/Licensed Practical Nurse in Florida. </li><li>Participates in continuing education so that he/she has knowledge of current trends and developments in nursing and maintains current license. </li><li>Minimum of two years of RAI and Care Plan experience, preferably in a Skilled Nursing Facility (SNF). </li><li>Must have knowledge of MDS 3.0 including Assessments, CAA's and Care Plans. </li><li>Conduct each Assessment and Care Plan with the appropriate participation of health professionals. </li><li>Ability to organize work procedures, assume responsibility, and tactfully interact with other disciplines. </li><li>Must be able to communicate well with any member of the Interdisciplinary Team. </li><li>Collaborate with the IDT to provide guidance on appropriate and precise charting for the medical record. </li></ul> <p>PERFORMANCE REQUIREMENTS:</p> <ul> <li>Must be able to sit, stand, bend, twist, lift and move intermittently throughout the day. </li><li>Must be able to lift, push, pull and otherwise move a minimum of 50 pounds. </li><li>Able to see and hear adequately in order to respond and interact with other staff and families. </li><li>Able to speak in clear, concise voice in order to communicate with patients, families, and other staff and disciplines. </li><li>Relate to and work with the elderly, disabled, emotionally upset and hostile Residents. </li><li>From time to time, you may be required to assume the duties of a floor nurse. </li></ul> <p>SPECIFIC DUTIES AND RESPONSIBILITIES:</p> <ul> <li>Assign responsibility for completing sections of the MDS to staff who have clinical knowledge about the patient. </li><li>Assure that non-licensed staff who provide direct care to the patients are allowed to contribute information for the assessment process. </li><li>Conducts, completes or coordinates other disciplines in completion of the RAI and certifies the completion of the Assessment. </li><li>Sign and certify the CAA Summary Form. </li><li>Attend care planning and enter the care plan decision information. </li><li>Assist with month-end duties for closeout. </li><li>Perform other assignments as assigned. </li></ul>
