<p>The STAR-H program will offer specialized housing support and wrap-around services to metro-Springfield area adults experiencing both chronic homelessness and co-occurring mental health and substance use disorders. With the University of Massachusetts Chan Medical School as the Local Lead Agency and Evaluator, the Behavioral Health Network, Inc., will be the clinical provider. Newly hired STAR-H Case Managers and Peer Support Specialists will attend Regional Network HUB meetings for referrals and deliver real-time Housing First and Maintaining Independence and Sobriety through Systems Integration, Outreach, and Networking (MISSION) wraparound services. MISSION is a multicomponent intervention that combines Housing First, Critical Time Intervention, Integrated Mental Health and Substance use Disorder Treatment, Peer Support, Vocational Support, Trauma Informed Care, and supports clients with COD medication needs. Participants will receive a rapid assessment, 12-months of housing support, and culturally responsive MISSION COD treatment and wraparound services. MISSION wraparound supports will also include key linkages to community-based, state, and federal mainstream benefit programs such as the MassHealth (state Medicaid authority), and the state Departments of Mental Health and Public Health services.</p> <ul> <li>Providing high-quality, community-based and clinically informed wrap-around services to help unhoused adults make positive changes through treatment. </li><li>Using sound clinical theories and principles in both individual and group work. </li><li>Providing both evidence-based co-occurring disorder intervention and wraparound supports, including clinical case management and community outreach. </li><li>Providing services in the community, in shelters, courts, clinics, and other settings. </li><li>Carrying a caseload of 15-20 adults who are chronically homeless </li><li>Building working relationships with agencies and organizations that address housing insecurity and support individuals' recovery to facilitate referral and care coordination. </li><li>Working closely with Springfield's Regional HUB Network, BHN's Project Hope Outreach Team, the Springfield District Arraignment Court wrap-around team, and other community stakeholders </li><li>Completing compliant, high-quality, and timely documentation </li><li>Increasing practice knowledge through supervision, training and development, seminars, and conferences. </li><li>Master's Degree in Clinical, Counseling Psychology or Social Work with 1-2 years post-Masters' experience working with adults with co-occurring mental health and substance use disorders (required). </li><li>Experience working with adults experiencing chronic homelessness (required). </li><li>Knowledge of psychotherapy, substance use and co-occurring disorders, and group work (required). </li><li>Theoretical knowledge and experience to provide case management, outreach and vocational support (required). </li><li>Working knowledge of community resources, and how to help individuals access them (required). </li><li>Experience conducting assessments with clinical formulations and recommendations based on sound clinical principles and judgment (required). </li><li>Experience doing risk/crisis evaluations, and taking appropriate actions to keep people served and others safe (required). </li><li>Ability to use electronic health records and other tech-enabled applications with facility (required). </li><li>Experience working in Massachusetts community-based mental health and substance use treatment programs (required). </li><li>Willingness and ability to treat challenging and diverse individuals and populations (required). </li><li>Experience running groups preferred </li><li>Bilingual in Spanish and English (preferred). </li></ul> <p>#LI-MD1</p>
