<p>Description</p> <p>The Transportation team in the Child Protection Services area is seeking a Case Management Assistant. In this role, the Case Management Assistant will be transporting children to appointments, visitations, shelters, and other destinations.</p> <p>Location and hours:</p> <p>This position is hybrid and will be performed onsite at the Richfield Service Center located at 6625 Lyndale Avenue South, Richfield, MN 55423, and remote as job duties require. Work hours are Monday through Friday, 11:30 a.m. - 8 p.m. (flexibility will be required to work outside of these hours and at times that will include evenings and weekends). While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.</p> <p>About this position type:</p> <p>This is a full-time benefit earning position. You must be a current regular or probationary employee of Hennepin County to apply for this position. Limited duration employees not eligible.</p> <p>This position is internally classified as a Case Management Assistant.</p> <p>Click here to view the job classification specification.</p> <p>In this position, you will:</p> <ul> <li>Safely transport children, 0-36 months, to and from appointments scheduled by Child Protection Social Workers. </li><li>Document case notes in Social Services Information System (SSIS) within 48 hours of the transport. </li><li>Collaborate with social workers, providers, and other professionals to ensure transportation needs are met. </li><li>Perform confidential case management. </li><li>Attend and participate in team meetings and all required trainings. </li></ul> <p>Need to have:</p> <ul> <li> <p>One of the following:</p> </li><li> <p>Two years or more of responsible, paid social service experience providing direct client service.</p> </li><li> <p>A two-year human services certificate.</p> </li><li> <p>Two years or more of college coursework in the behavioral sciences.</p> </li><li> <p>A valid driver's license, access to reliable transportation, and the ability to obtain a Hennepin County driver's permit to use a personal vehicle to travel to various shelters, drop-in centers throughout Hennepin County, and transport clients when needed.</p> </li></ul> <p>Nice to have:</p> <ul> <li> <p>Experience:</p> </li><li> <p>Working with child protection programs.</p> </li><li> <p>Working with databases and computer programs to perform data entry where accuracy is essential.</p> </li><li> <p>Knowledge of community resources and social services available to clients.</p> </li><li> <p>Ability to:</p> </li><li> <p>Communicate with individuals from a variety of cultural and lifestyle backgrounds in a respectful and non-judgmental manner.</p> </li><li> <p>Work effectively with external and internal partners.</p> </li></ul> <p>Candidates with fluency in another language in addition to English are encouraged to apply.
