Our holistic approach includes OBAT, IOP, and mobile treatment units, with a strong focus on patient-centered, evidence-based recovery and community partnerships.</p> <p>Your Role:</p> <p>The Health Homes Coordinator oversees daily nursing operations, manages case loads, ensures compliance with state and federal regulations, supervises staff, and supports quality patient care in an outpatient opioid treatment program (OTP).</p> <p>Key Responsibilities:</p> <ul> <li>Supervising case managers </li><li>Manage case loads and ensure high-quality, coordinated patient care </li><li>Lead quality assurance and compliance activities </li><li>Oversee medical record documentation </li><li>Collaborate with internal teams and external community partners </li><li>Engage in community outreach and resource coordination </li></ul> <p>Compensation for roles at Woonsocket CTC varies depending on a wide array of factors, including but not limited to the specific location, role, skill set, and level of experience.
