<p>Title: Care Coordinator-Employee Assistance Program</p> <p>Location: Upper East Side</p> <p>Org Unit: CopeNYP EAP Program</p> <p>Work Days:</p> <p>Weekly Hours: 35.00</p> <p>Exemption Status: Exempt</p> <p>Salary Range: $73,600.00 - $85,000.00</p> <ul> <li>As required under NYC Human Rights Law Int 1208-2018 - Salary range for this role when Hired for NYC Offices </li></ul> <p>Position Summary</p> <p>Provides telephonic intake assessments, makes referrals to the Employee Assistant Program's (EAP) counselors and other company-sponsored benefits, coordinates referrals to longer-term care for the EAP Team, creates wellness content for EAP training programs, and performs other administrative functions as requested by the Clinical Director.</p> <p>Job Responsibilities</p> <ul> <li>Conducts telephonic intakes with client for the EAP; responds to messages relayed from other EAP and back-up call center staff. </li><li>Documents all client contacts in a thorough and timely manner in EMR (electronic medical records) software. </li><li>Documents non-clinical contacts and services (e.g., management consultations, critical incident requests, wellness training requests and program attendance, satisfaction and outcome survey results, etc.) in Excel or other software as appropriate. </li><li>Manages and charts full-time and part-time staff schedules. </li><li>Obtains and charts data for utilization and budget reports and flow charts using Excel, PowerPoint, Visio, and/or EAP software. </li><li>Researches and creates content for wellness webinars and develops associated PowerPoint presentations. </li><li>Provides referrals to EAP clinicians. </li><li>Assists EAP staff in making longer-term care or higher level-of-care referrals for clients utilizing clinicians and programs who are in-network with the client's behavioral health insurance, specialty care and/or community resources. </li><li>Provides management consultations to HR, managers, supervisor and leadership around troubled employees, employees with performance issues, and around initial Critical Incident Response (CIR) requests. </li><li>Provides referrals to all relevant non-EAP company-sponsored benefits (legal, financial, child-care, work-life, Nurseline, smoking cessation, etc.). </li><li>Attends and contributes to team meetings and supervisory and peer supervision. </li></ul> <p>Education</p> <ul> <li>Master's Degree </li></ul> <p>Experience</p> <p>LMSW, LMHC or other entry-level licensed Master's-level clinician.</p> <p>Experience with principles of Care Management and Care Coordination a plus.</p> <p>Knowledge, Skills and Abilities</p> <ul> <li>Basic to moderate expertise in Word, Excel, and PowerPoint. </li><li>Ability to master EAP software and other relevant software as needed (e.g., Visio). </li><li>Strong customer service focus with ability to appropriately manage escalations; communicates effectively with clients and staff. </li><li>Strong organizational skills, including tracking multiple staff calendars and analyzing supply/demand issues relative to staffing levels. </li><li>Ability to research wellness/psychology/relationship topics and create content for webinars & seminars based on research from reputable and documented source material in PowerPoint. </li><li>Ability and willingness to take on projects as needed as assigned by the Clinical Director or other program leaders. </li></ul> <p>Licenses and Certifications</p> <ul> <li>Dependent on candidate's qualifications. </li></ul> <p>Working Conditions/Physical Demands</p> <p>Cornell welcomes students, faculty, and staff with diverse backgrounds from across the globe to pursue world-class education and career opportunities, to further the founding principle of "any person, any study."
