<p>Primary Purpose:</p> <p>As a leader of the Patient Centered Medical Home Team, this position will provide healthcare to adult and pediatric patients by leading a care management team using the patient-centered medical home team model. This person will also assist in clinic teaching and fulfill administrative responsibilities.</p> <p>The person in this position is a member of the Patient Centered Health Care Home team where each team member plays an integral role in providing patient-centered health care.</p> <p>This position's starting salary is $200,000.</p> <p>Essential Duties:</p> <ul> <li>Practice and provide care in accordance with acceptable standards of Family Medicine, the Clinical Practice Guidelines of KPHC, and other written policies and standing orders of KPHC. </li><li>Provide comprehensive patient care with the ability to consult and/or refer to other health care providers/services/agencies as necessary. </li><li>Order and interpret laboratory tests, studies and ancillary services. </li><li>Prescribe medical treatment and clinical drugs to patients. </li><li>Perform approved therapeutic procedures. </li><li>Dispense medications from within clinic supply in accordance with legal and clinic standards. </li><li>Maintain charting in accordance with legal and clinic standards. </li><li>Instruct and advise patients regarding preventive health care measures to promote physical and mental well-being and to promote early detection, diagnosis and management of medical conditions. </li><li>May be required to supervise Physician's Assistants or other staff as assigned. </li><li>Review and manage on a regular basis chronic care cases that require ongoing medical attention. </li><li>Provide/promote "informed consent" and knowledgeable participation of clients in their care and in preventive care. </li><li>Maintain confidentiality of all patient information according to federal guidelines and regulations. </li><li>Assist in the resolution of complaints, requests and inquiries from patients. </li><li>Provide health education/consultation to clients, families, other professionals and the community as appropriate. </li><li>Collaborate in planning and participate in teaching paraprofessionals, medical students, residents, nursing students and other health care professionals as assigned. </li><li>Expected to attend All-Staff Meetings bi-monthly and other staff and professional meetings as required. </li><li>Participate in health center and professional quality assurance plans/activities, data collection and analysis. </li><li>Participate in monthly peer review, support staff evaluation, case conferences, hospital committees and other activities as required. </li><li>Assist in evaluating and revising Clinical Practice Guidelines for practice in the clinic and in other practice policies and procedures. </li><li>Provide after-hours call coverage shared with other members of the adult medicine team. </li><li>May be required to provide outpatient care at one or more KPHC sites. </li><li>Communicate concerns and suggestions to the Medical Director, Clinical Operations Director and to other medical team providers verbally and in writing, where appropriate. </li></ul> <p>Regular and reliable on-site attendance is required.</p> <p>Other Duties:</p> <ul> <li>Perform other related duties as assigned. </li></ul> <p>Minimum Qualification Requirements:</p> <p>Education:</p> <ul> <li>Medical or Osteopathic doctorate degree from an accredited medical school required. </li><li>Completion of residency in Family Medicine at an accredited US residency program required. </li><li>Required to participate in continuing education programs and maintain current BLS certification. </li></ul> <p>Licensing/Certification</p> <ul> <li>Unrestricted license to practice medicine in the State of Hawaii required </li><li>Board Eligibility or Board Certification in Family Medicine required </li><li>Current Basic Life Support certification required. </li><li>Current DEA and Hawaii State Narcotics registration required. </li><li>Must be eligible for credentialing with Quest and the majority of other third party payers with whom KPHC participates. </li></ul> <p>Skills/Knowledge:</p> <ul> <li>Must be able to work with multi-ethnic and multi-cultural staff and clients. </li><li>Knowledge of and ability to apply professional medical principles, procedures, and techniques. </li><li>Thorough knowledge of pharmacological agents used in patient treatment. </li><li>Effective verbal and written communication skills along with proper telephone etiquette. </li><li>Perform in a tactful and professional manner. </li></ul> <p>BENEFITS</p> <p>KPHC provides four employer-sponsored health insurance plans to choose from (one Kaiser & three HMSA). KPHC pays 100% of employee single coverage health insurance for the base CompMed HMSA plan or KP 301 plan. All plans include medical, drug, vision, dental, and chiropractic coverage.</p> <ul> <li>Generous Holiday Schedule: KPHC observes twelve (12) full holidays and two (2) holiday half days all paid as full day. </li><li>PTO: In the first year, a full-time staff can accumulate up to 138 hours of PTO. PTO is paid out upon separation with company when notice is given. </li><li>Educational Assistance: KPHC may provide financial assistance to employees who of their own accord chose to pursue advancement of knowledge and skills through a course of study or to assist in the repayment of certain loans where a higher degree has already been obtained, if it is applicable to their position at KPHC. All KPHC eligible employees will have the opportunity to receive available funds by applying and receiving approval for Educational Assistance or Tuition Reimbursement. </li><li>Loan Forgiveness: KPHC is a qualifying employer for the Public Service Loan Forgiveness Program and the Hawaii State Loan Repayment Program. </li><li>Retirement: KPHC has a safe harbor contribution of 3% of employee's annual compensation toward Tax Sheltered Annuity after the employee has completed a calendar year of service. Employees are 100% vested in retirement plan. </li><li>Life Insurance Policy: KPHC pays 100% for Employee Group Life Insurance and AD&D insurance of a maximum of $50K coverage </li><li>Flexible Spending Plans: KPHC employees will be able to participate in the FSA plan from the inception of employment. </li><li>Transportation Benefit: Eligible employees can receive a HOLO bus pass paid by KPHC. </li><li>Bereavement Leave: Eligible employees receive up to five days of bereavement leave. </li><li>Jury Duty: KPHC will pay the difference between the employee's regular pay and the payment received for jury duty for maximum of five (5) days. </li><li>Employee Assistance Program: KPHC provides an EAP designed to provide free and confidential support, resources and information for personal and work issues.
