This role collaborates across disciplines to ensure the development and implementation of individualized care plans that meet patient needs and regulatory standards.</p> <p>Essential Functions:</p> <ul> <li> <p>Complete departmental orientation as well as initial and annual competencies.</p> </li><li> <p>Collaborate with interdisciplinary staff to identify and resolve barriers to care or discharge.</p> </li><li> <p>Complete documentation in accordance with workflow timelines and content standards, including the Individual Plan of Care (IPoC) per CMS guidelines.</p> </li><li> <p>Schedule and conduct family conferences and caregiver communications following team meetings or as needed.</p> </li><li> <p>Facilitate weekly care team conferences to review patient progress and adjust treatment plans.</p> </li><li> <p>Review the assigned Case Mix Group (CMG) and assist the care team in identifying and documenting treated comorbid conditions; notify the Health Information Management (HIM) team as needed.</p> </li></ul> <p>Qualifications:</p> <ul> <li> <p>Education: Bachelor's or Master's degree in Social Work, or as required by state regulations</p> </li><li> <p>Certifications: Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ACM, RN-BC, ARN).</p> </li><li> <p>Experience: Minimum of 2 years of social work or case management experience in an inpatient setting highly preferred; experience in acute or rehabilitation hospital preferred.</p> </li></ul> <p>EEOC Statement</p> <p>We are an Equal Opportunity Employer.
